Care Coordinator – Hybrid RN/PT/OT/ST (Eugene, OR)

Job Summary

Optum Home & Community Care is hiring a Care Coordinator (RN, PT, OT, or ST) to support post-acute care (PAC) transitions, discharge planning, and patient recovery optimization. This role involves coordinating care between healthcare teams, patients, and families to ensure smooth transitions from hospital to home. Ideal candidates have a strong clinical background and passion for improving senior patient outcomes.


Key Responsibilities

  • Serve as the main liaison between patients and healthcare teams for smooth care transitions.

  • Conduct Skilled Nursing Facility (SNF) assessments using CMS criteria.

  • Review patient outcomes and discharge plans with healthcare providers and families.

  • Complete SNF concurrent reviews and update authorizations on time.

  • Collaborate with physicians, discharge planners, social workers, and therapists for optimal discharge planning.

  • Participate in SNF rounds and ensure appropriate referrals to case managers or community services.

  • Monitor patient progress toward discharge goals and resolve care barriers.

  • Maintain accurate and timely documentation in coordination systems.

  • Attend patient/family care conferences and H&C Transitions meetings.

  • Adhere to HIPAA and federal/state regulations on PHI confidentiality.

  • Support contract start-ups and manage caseloads effectively.


Required Skills & Qualifications

  • Active, unrestricted clinical license in Oregon (RN, PT, OT, or Speech Language Pathologist).

  • 5+ years of clinical experience in healthcare settings.

  • Residency within or near Eugene, OR, or within 30 miles of the listed facilities.

  • Strong knowledge of CMS guidelines and care coordination processes.

  • Proficiency in Microsoft Office (Outlook, Excel, PowerPoint).

  • Excellent communication, problem-solving, and time-management skills.

Preferred Qualifications

  • Experience with geriatrics, home health nursing, or rehabilitation.

  • Familiarity with care management and disease management programs.

  • Background in patient education and utilization/resource management.
