Job Summary:

UnitedHealth Group’s Optum Home & Community Care division is hiring a Care Coordinator (RN/PT/OT/ST) in South Bend, IN. This hybrid role focuses on care transitions, post-acute care coordination, and patient engagement to improve health outcomes. With a competitive pay range of $34.23 to $61.15 per hour, this opportunity is ideal for experienced healthcare professionals seeking impactful roles in clinical operations.


Key Responsibilities:

  • Perform Skilled Nursing Facility (SNF) assessments and review discharge plans.

  • Collaborate with interdisciplinary healthcare teams to ensure optimal patient outcomes.

  • Conduct weekly SNF rounds and provide accurate updates to medical directors.

  • Engage patients and families telephonically or onsite to facilitate care transitions.

  • Ensure timely and accurate documentation in workflow systems.

  • Coordinate peer-to-peer reviews and assist with utilization management.

  • Identify barriers to discharge and recommend care management solutions.

  • Adhere to HIPAA compliance and maintain patient confidentiality.


Required Skills & Qualifications:

  • Active and unrestricted clinical license in Indiana – RN, PT, OT, or Speech Language Pathologist.

  • 5+ years of clinical experience in healthcare settings.

  • Strong understanding of care management, SNF processes, and CMS guidelines.

  • Proficiency in Microsoft Office Suite (Outlook, Excel, PowerPoint).

  • Excellent communication, problem-solving, and conflict-resolution skills.

  • Valid driver’s license and reliable transportation (for local facility visits).


Preferred Qualifications:

  • Experience with geriatric care, rehabilitation, or home health nursing.

  • Familiarity with utilization/resource management and disease management programs.

  • Strong team collaboration and multi-tasking abilities.
