Job Summary

Optum Home & Community Care, part of UnitedHealth Group, is seeking an experienced Care Coordinator to support post-acute care transitions and optimize patient recovery. This hybrid position combines onsite facility coordination with remote administrative tasks, enabling healthcare professionals to engage directly with patients, families, and interdisciplinary care teams. The role is ideal for Registered Nurses (RN), Physical Therapists (PT), Occupational Therapists (OT), or Speech Therapists (ST) with strong care management experience and expertise in discharge planning.


Key Responsibilities

  • Conduct Skilled Nursing Facility (SNF) assessments upon admission and throughout patient stays using CMS criteria.

  • Collaborate with interdisciplinary teams (physicians, discharge planners, therapists, social workers) to create optimal discharge plans.

  • Track and support patient progress toward recovery and resolve any care barriers.

  • Participate in weekly SNF rounds and family care conferences to ensure updated care plans.

  • Manage authorizations and concurrent reviews for SNF stays.

  • Coordinate peer-to-peer reviews with medical directors and escalate complex cases as required.

  • Maintain accurate documentation and caseload management, ensuring compliance with policies.

  • Monitor care outcomes through dashboards, reports, and readmission data to identify process improvements.

  • Engage with patients, families, and caregivers onsite or telephonically to ensure informed decision-making.


Required Skills & Qualifications

  • Active, unrestricted clinical license (RN, PT, OT, or ST) in the state of hire.

  • Minimum of 5 years of clinical experience in healthcare.

  • Valid driver’s license and reliable transportation (within a 30-mile radius of Bend, OR).

  • Strong time management, documentation, and care coordination skills.

Preferred Qualifications

  • Experience with geriatric populations, rehabilitation, or home health nursing.

  • Familiarity with utilization management, care management, and disease management programs.

  • Proficiency with Microsoft Office (Outlook, Excel, PowerPoint).

  • Exceptional communication, problem-solving, and conflict resolution skills.
