Job Summary:

The Pharmacist provides pharmaceutical care with a focus on clinical monitoring and therapeutic interventions to improve patient outcomes. Responsibilities include verifying medication orders, ensuring dispensing accuracy, monitoring therapy effectiveness, and supervising pharmacy staff. The role also involves participation in performance improvement initiatives, medication safety, and compliance with professional standards.

Job Duties:

1. Medication Order Verification

  • Review and interpret medication orders from licensed prescribers.
  • Ensure completeness and appropriateness of medication orders.
  • Assess drug interactions, allergies, therapy duplication, and potential interactions.
  • Clarify incomplete or suboptimal orders with prescribers and recommend therapeutic alternatives.
  • Perform secondary verification for pediatric and chemotherapy orders.

2. Therapeutic Clinical Monitoring

  • Monitor ongoing medication therapy per approved Collaborative Care Agreements.
  • Provide therapeutic recommendations and optimize patient care.
  • Document assessments and recommendations in patient records.
  • Communicate pharmacy-related interventions with the interdisciplinary team.

3. Compounding and Dispensing Pharmaceuticals

  • Ensure timely dispensing of accurate medications.
  • Supervise compounding per USP standards, verifying ingredient compatibility and labeling.
  • Check all medications before final dispensing, including those for automated dispensing cabinets and floor stock.
  • Utilize barcode technology for accurate dispensing.
  • Oversee repackaging, ensuring proper documentation of lot numbers and expiration dates.

4. Supervision of Pharmacy Interns and Technicians

  • Provide active supervision and mentoring in alignment with ICARE values.
  • Monitor task completion and redirect staff as needed.
  • Promote professional interaction using the AIDET model.
  • Report unresponsive behavior to leadership.

5. Medication Use Safety Initiatives

  • Detect and report dispensing variances.
  • Identify opportunities to enhance medication safety (e.g., clear labeling, system improvements).
  • Report adverse drug reactions.

6. Controlled Substance Accountability

  • Supervise receipt, dispensing, and distribution of controlled substances.
  • Ensure accurate inventories and reconcile discrepancies.
  • Manage controlled substance reports and verify proper documentation.

7. Financial Stewardship

  • Encourage formulary compliance and therapeutic interchanges.
  • Process non-formulary requests appropriately.
  • Suggest therapeutic alternatives and implement IV to PO conversions.
  • Manage time effectively to minimize overtime.

8. Drug Information & Staff Development

  • Provide accurate and timely drug information to medical and nursing staff.
  • Act as a preceptor for Pharmacy Residents and Interns.
  • Develop and conduct staff education programs.

9. Professional Competency

  • Maintain active pharmacist licensure.
  • Complete all required competency assessments.
  • Stay updated on current pharmacy practices and standards.
  • Develop specialized practice skills.

10. Additional Responsibilities

  • Perform other duties as assigned.

Qualifications:

Education & Credentials:

  • Graduate from an ACPE-accredited pharmacy college.
  • Current U.S. pharmacist license in the state of practice.

Specialized Knowledge & Skills:

  • Strong problem-solving and critical-thinking skills.
  • Effective communication skills.
  • Familiarity with current practice standards (ASHP).

Experience:

  • Previous hospital pharmacy experience preferred.
