Pharmacist Float Pool - St. Elizabeth Healthcare

35 hours per week (Third Shift - 7 On/7 Off)


Job Summary:

The Pharmacist Float Pool at St. Elizabeth Healthcare provides pharmaceutical care to patients across various acute care facilities, offering flexibility and support where it's most needed. The pharmacist will work on all shifts and at multiple sites to optimize medication therapies, improve patient outcomes, and support the hospital's medication use processes. This position offers premium pay and bonuses as an added incentive.


Key Responsibilities:

  • Medication Order Verification:

    • Review and interpret medication orders, ensuring accuracy and appropriateness based on the patient's condition.
    • Assess drug interactions, allergies, and potential therapeutic issues.
    • Provide prescribers with therapeutic alternatives when necessary.
  • Therapeutic Clinical Monitoring:

    • Actively monitor ongoing medication therapy in compliance with collaborative care agreements and protocols.
    • Document medication assessments and interventions in the patient’s chart and keep the interdisciplinary team informed.
  • Compounding and Dispensing Pharmaceuticals:

    • Ensure timely dispensing and proper compounding of medications according to USP standards.
    • Supervise verification of medication content, labeling, and ingredient compatibility.
    • Use barcode technology to ensure accurate dispensing.
  • Supervision of Interns and Technicians:

    • Mentor and supervise pharmacy interns and technicians.
    • Ensure timely completion of tasks and provide redirection when necessary.
  • Medication Safety Initiatives:

    • Report medication variances and adverse drug reactions.
    • Participate in efforts to improve the safe use of medications and medication management systems.
  • Controlled Substance Accountability:

    • Oversee the receipt, dispensing, and distribution of controlled substances using Pyxis and other inventory management systems.
    • Ensure accuracy of inventories and resolve discrepancies.
  • Financial Management:

    • Encourage prescriber compliance with formulary restrictions and therapeutic interchanges.
    • Participate in IV to PO conversions and suggest therapeutic alternatives for non-formulary requests.
  • Drug Information & Staff Development:

    • Provide drug information to medical and nursing staff, as well as patients.
    • Serve as a preceptor for pharmacy residents and interns, and develop staff education programs.
  • Professional Competency:

    • Maintain licensure and complete departmental competency assessments.
    • Stay updated on pharmacy practices and continue developing specialized skills.

Education & Credentials:

  • Graduate from an ACPE-accredited pharmacy college.
  • Current pharmacist licensure in Kentucky and Indiana (or eligibility for licensure in both).

Specialized Knowledge & Skills:

  • Strong problem-solving and critical thinking skills.
  • Excellent communication skills.
  • Familiarity with current pharmacy practice standards (ASHP).

Experience:

  • At least 6 months of pharmacist experience in a hospital or comparable healthcare setting.
