This opportunity is ideal for a licensed pharmacist seeking a high-impact inpatient IV admixture services (IVAS) role within a collaborative, multidisciplinary hospital environment.


Role Overview

The Inpatient IVAS Pharmacist is responsible for the preparation, verification, and dispensing of sterile and non-sterile pharmaceutical products within the hospital inpatient pharmacy setting. This position plays a critical role in ensuring medication safety, optimizing therapeutic outcomes, and supporting clinical decision-making across inpatient units.

The pharmacist collaborates closely with physicians, nurses, and allied healthcare professionals to prevent medication errors, ensure regulatory compliance, and deliver advanced clinical pharmacy services.


Key Responsibilities

  • Prepare, verify, and dispense sterile compounded IV medications in accordance with USP <797> and institutional standards

  • Review medication orders for accuracy, appropriateness, and therapeutic optimization

  • Provide pharmacokinetic dosing recommendations and therapeutic drug monitoring

  • Conduct pharmacist-led interventions to improve medication safety and patient outcomes

  • Implement IV to PO conversions where clinically appropriate

  • Manage Total Parenteral Nutrition (TPN) under pharmacy-driven protocols

  • Participate in therapeutic interchange programs

  • Ensure compliance with hospital policies, state regulations, and accreditation standards

  • Collaborate with multidisciplinary healthcare teams to prevent inappropriate or misuse of medications


Required Qualifications

  • Doctor of Pharmacy (PharmD) or equivalent degree

  • Active and unrestricted Pharmacist license in the State of California

  • Minimum 2 years of inpatient hospital pharmacy experience preferred

  • Experience in IV admixture services (IVAS) or sterile compounding strongly preferred

  • Knowledge of pharmacokinetics, antimicrobial stewardship, and clinical dosing protocols

  • Strong clinical judgment and communication skills

  • Ability to work variable evening shifts, weekends, and holidays


Preferred Qualifications

  • PGY1 Pharmacy Residency (highly preferred)

  • Experience in academic medical center or tertiary care hospital

  • Board Certification (e.g., BCPS) preferred but not mandatory
