Role Overview
The Inpatient IVAS Pharmacist is responsible for the preparation, verification, and dispensing of sterile and non-sterile pharmaceutical products within the hospital inpatient pharmacy setting. This position plays a critical role in ensuring medication safety, optimizing therapeutic outcomes, and supporting clinical decision-making across inpatient units.
The pharmacist collaborates closely with physicians, nurses, and allied healthcare professionals to prevent medication errors, ensure regulatory compliance, and deliver advanced clinical pharmacy services.
Key Responsibilities
Prepare, verify, and dispense sterile compounded IV medications in accordance with USP <797> and institutional standards
Review medication orders for accuracy, appropriateness, and therapeutic optimization
Provide pharmacokinetic dosing recommendations and therapeutic drug monitoring
Conduct pharmacist-led interventions to improve medication safety and patient outcomes
Implement IV to PO conversions where clinically appropriate
Manage Total Parenteral Nutrition (TPN) under pharmacy-driven protocols
Participate in therapeutic interchange programs
Ensure compliance with hospital policies, state regulations, and accreditation standards
Collaborate with multidisciplinary healthcare teams to prevent inappropriate or misuse of medications
Required Qualifications
Doctor of Pharmacy (PharmD) or equivalent degree
Active and unrestricted Pharmacist license in the State of California
Minimum 2 years of inpatient hospital pharmacy experience preferred
Experience in IV admixture services (IVAS) or sterile compounding strongly preferred
Knowledge of pharmacokinetics, antimicrobial stewardship, and clinical dosing protocols
Strong clinical judgment and communication skills
Ability to work variable evening shifts, weekends, and holidays
Preferred Qualifications
PGY1 Pharmacy Residency (highly preferred)
Experience in academic medical center or tertiary care hospital
Board Certification (e.g., BCPS) preferred but not mandatory
