About the Role:
UCLA Health is seeking a dedicated Per Diem Pharmacist to join the Pharmacy team at UCLA West Valley Medical Center. This role is critical in ensuring safe, accurate, and efficient medication management across inpatient and medication reconciliation departments. As a floating pharmacist, you will provide clinical services, support interdisciplinary teams, and help optimize patient outcomes through expert pharmaceutical care.

Key Responsibilities:

  • Prepare and dispense a wide range of medications accurately and efficiently in the inpatient pharmacy.

  • Float between medication reconciliation and inpatient pharmacy departments to ensure seamless medication management.

  • Collaborate with physicians, nurses, and allied health professionals to prevent inappropriate or unsafe medication use.

  • Provide clinical pharmacy services including pharmacokinetic dosing, therapeutic interchange, IV/PO conversions, TPN management, and pharmacist interventions.

  • Adhere to departmental policies, procedures, and regulatory standards to maintain high-quality patient care.

Required Qualifications:

  • Bachelor of Pharmacy (B.Pharm) or Doctor of Pharmacy (PharmD) degree from an accredited institution.

  • Active pharmacist licensure in the state of California (or eligibility to obtain licensure).

  • Strong knowledge of medication therapy management, drug interactions, and clinical pharmacy practices.

  • Ability to work flexible shifts, including weekends and holidays, as needed by departmental operations.

  • Excellent communication, teamwork, and problem-solving skills.

Preferred Qualifications:

  • Previous hospital pharmacy experience, particularly in inpatient or clinical pharmacy services.

  • Familiarity with electronic medical records (EMR) and pharmacy information systems.

  • Experience with clinical interventions and patient-focused pharmacy services.
