Position Overview

The Inpatient IVAS Pharmacist plays a vital role in supporting inpatient pharmacy operations through the preparation, verification, and dispensing of intravenous and other sterile medications. This position delivers critical clinical pharmacy services while collaborating closely with multidisciplinary healthcare teams to promote safe, effective, and evidence-based medication use in the inpatient setting.

This role is ideal for pharmacists with strong clinical judgment and experience in sterile compounding and inpatient medication management.


Key Responsibilities

  • Prepare, verify, and dispense sterile and non-sterile medications for inpatient use in compliance with IVAS standards

  • Deliver comprehensive inpatient clinical pharmacy services, including pharmacokinetic dosing, IV-to-PO conversions, therapeutic interchange, and total parenteral nutrition (TPN) per pharmacy protocols

  • Collaborate with physicians, nurses, and allied healthcare professionals to prevent medication errors and inappropriate medication use

  • Perform clinical interventions to optimize patient outcomes and ensure medication safety

  • Adhere strictly to departmental policies, regulatory requirements, and hospital pharmacy standards

  • Support continuous quality improvement initiatives within inpatient pharmacy operations


Required Qualifications & Experience

Education & Licensure:

  • Doctor of Pharmacy (PharmD) degree from an accredited institution

  • Active pharmacist license in the state of California (or eligibility to obtain)

Experience:

  • Minimum 1–3 years of experience in inpatient hospital pharmacy or IV admixture services preferred

  • Experience with sterile compounding, IV admixture preparation, and inpatient medication distribution

  • Familiarity with USP <797> and USP <800> standards strongly preferred

Skills & Competencies:

  • Strong clinical decision-making and problem-solving skills

  • Ability to work effectively during evening, weekend, and holiday shifts

  • Excellent communication and interdisciplinary collaboration abilities

  • Commitment to patient safety, quality improvement, and regulatory compliance
