Position Summary:

As a Field-Based Patient Care Coordinator, you will support nurse practitioners (NPs) and physician assistants (PAs) in the coordination of care and administrative tasks, directly contributing to patient well-being. You will serve as a key liaison between the care team and patients in Orlando, FL and nearby communities. This is a full-time, Monday–Friday (8:00 AM – 5:00 PM) position with no nights, weekends, or holidays.


Primary Responsibilities:

  • Coordinate care activities under the guidance of NPs, PAs, and RNs

  • Update patient records for new admissions, post-hospital care, labs, medications, and quality metrics

  • Obtain medical records from external providers

  • Schedule patient appointments and transportation

Required Qualifications:

  • High School Diploma / GED or equivalent experience

  • 18 years of age or older

  • Minimum 3 years of clinical or medical record experience

  • Familiarity with medical terminology and EHR systems

  • Reliable transportation and willingness to travel up to 100% locally

  • Must reside in Orlando, FL or surrounding areas

  • Full-time availability: Monday–Friday, 8:00 AM – 5:00 PM


Preferred Qualifications:

  • Certified Nursing Assistant (CNA) or Medical Assistant (MA) certification
