UnitedHealthcare is seeking a Field Service Coordinator to support Skilled Nursing Facility (SNF) operations and coordinate care for dual-eligible members across Ada County, Idaho. This role combines remote work flexibility with field-based member engagement to improve health outcomes, reduce care gaps, and promote community reintegration.

Candidates must reside within 50 miles of Ada County and should be able to travel across the assigned territory based on member and business needs.


Position Overview

The Field Service Coordinator serves as the primary care manager for members with Medicare/Medicaid coverage. The role focuses on comprehensive assessments, individualized care planning, transitional care coordination, and connecting members with essential clinical and community services.


Key Responsibilities

  • Serve as the main care manager for dual-eligible members across assigned regions.

  • Conduct comprehensive medical, functional, behavioral, cultural, and social needs assessments through in-person and telephonic interactions.

  • Develop and implement personalized care plans focusing on individual goals, readiness for change, and quality-of-life improvement.

  • Collaborate closely with providers, internal care teams, and community-based resources to execute care plans effectively.

  • Assist members in accessing Home and Community-Based Services (HCBS).

  • Facilitate referrals to internal specialists (housing, pharmacy, peer support) and external providers such as PCPs, specialists, and behavioral health services.

  • Support proactive discharge planning and coordinate care transitions after ER visits, hospitalizations, or SNF stays.

  • Educate and coach members to enhance self-management and adherence to care recommendations.

  • Advocate for member needs and ensure their choices are represented in all care decisions.

  • Respond to non-clinical provider and facility queries and guide them to relevant plan resources.


Required Qualifications

  • Undergraduate degree or equivalent professional experience.

  • Current and unrestricted Idaho license in one of the following:
    RN, LCSW, LMSW, LSW, LBSW, LCPC, LPC, LMFT, LAMFT

  • Minimum 1 year of experience working with Medicaid/Medicare populations or individuals with significant Social Drivers of Health (SDoH) needs.

  • Minimum 1 year of experience using Microsoft Word, Excel, and Outlook.

  • Ability to work from a secure, private home workspace with PHI protection.

  • Residence within an area eligible for UnitedHealth Group–approved high-speed internet.

  • Valid U.S. driver’s license and reliable transportation.

  • Ability to travel within the assigned territory up to 75% of the time.

  • Must reside within 50 miles of Ada County, Idaho.


Experience Required (Summary Section)

  • 1+ years working with Medicaid/Medicare or high SDoH–needs populations

  • 1+ years in clinical or social services environment

  • Previous experience in care coordination, case management, or community health preferred

  • Strong documentation and assessment experience using digital tools


Compensation and Benefits

Pay is determined by local labor markets, experience, education, and certifications.
Hourly Range: $28.27 – $50.48 (based on full-time employment)
