Position Overview:
As a Clinical Pharmacist, you will deliver patient-centered pharmaceutical care by providing medication information, education, preparation, and distribution. Your role will ensure safe, effective, and cost-efficient medication therapy. You will use your clinical judgment and skills to implement patient care plans based on diagnoses in a timely manner. By performing competent patient assessments and maintaining effective communication, you will gain the trust and cooperation of patients, their families, and other healthcare professionals. You will adhere to all regulations and policies to ensure privacy and safety while delivering optimal care. Responsibilities may also include performing specific tasks and orienting other staff in the department.
Job Description:
EDUCATION:
Doctorate in Pharmacy from an accredited institution.
CERTIFICATION & LICENSURE:
PHARMD: Licensed Pharmacist
OR PHARMR: Registered Pharmacist
ACLS: Advanced Cardiac Life Support (required for those responding to codes)
TYPICAL EXPERIENCE:
Minimum of 1 year of recent relevant experience.
SKILLS AND KNOWLEDGE:
Pharmaceutical Knowledge: Familiarity with medical terminology, generic and trade names, pharmaceutical calculations, and relevant laws and regulations.
Pharmacy Practice Settings: Understanding of narcotic delivery procedures and different pharmacy practice environments.
Legal Knowledge: Basic working knowledge of legal requirements and accreditation standards (NABP, TJC, Title XXII, DHS, DEA, FDA, USP).
Communication Skills: Ability to explain sensitive information clearly to diverse audiences, including non-medical personnel.
Time Management: Well-developed organizational skills with the ability to prioritize tasks effectively in a dynamic environment.
Technical Skills: General knowledge of computer applications (Microsoft Office Suite, EHR, EPIC).
Independence and Teamwork: Ability to work independently and collaboratively within a team, managing multiple tasks amid interruptions.
Problem Solving: Capacity to identify, evaluate, and resolve standard problems using established solutions.
Patient Privacy: Commitment to maintaining the privacy of patient health information (PHI).
