Care Coordinator (Referral Coordinator) – New Port Richey, FL

Location: On-site, 4759 US Highway 19, New Port Richey, FL 34652
Organization: Optum (UnitedHealth Group)
Schedule: Monday–Friday, 8:00 AM – 5:00 PM (occasional overtime)
Employment Type: Full-time
Compensation / Salary (clearly stated): $17.74 – $31.63 per hour (based on full-time employment; dependent on market, education, experience, certifications)
Training: 4–6 weeks paid, on-site


Job Summary

Optum is seeking a Care Coordinator (Referral Coordinator) to manage end-to-end referral processing, care coordination, scheduling, and medical management data entry. Working closely with providers, clinical teams, and patients, you will ensure timely authorizations, accurate documentation, HIPAA-compliant communication, and seamless access to specialty services. This role is ideal for healthcare administration professionals with strong customer service, EMR, and multi-tasking capabilities who want to grow within a mission-driven, equity-focused healthcare leader.


Key Responsibilities

  • Coordinate and process all patient referrals (specialty services, hospital, outpatient, DME) according to company SOPs.

  • Authorize and enter referral/service approvals in the system accurately and on time.

  • Schedule appointments, communicate timelines, and relay pertinent information to patients.

  • Verify and update patient demographics, insurance, and eligibility details.

  • Track and follow up on outstanding referral reports to ensure timely receipt and routing to the appropriate physician.

  • Maintain strict HIPAA compliance and protect PHI in all communications.

  • Deliver excellent customer service with a focus on quality, accuracy, and efficiency.

  • Support clinic operations with additional administrative duties as assigned.


Required Skills & Qualifications

  • High School Diploma/GED (or higher).

  • 2+ years of customer service experience in the healthcare field.

  • Intermediate proficiency in Microsoft Word, Excel, Outlook.

  • Strong telephone etiquette, written and verbal communication skills.

  • Ability to multi-task, prioritize, and meet deadlines in a high-paced environment.

  • Knowledge of HIPAA regulations and commitment to confidentiality.

  • 18 years of age or older.

Preferred Qualifications

  • Familiarity with diagnosis and CPT codes.

  • Prior exposure to EMR systems.

  • Knowledge of medical terminology and insurance products.

  • Bilingual fluency is a plus.
