Position Summary

The RN Combination Clinical Care Manager plays a central role in UnitedHealthcare’s Integrated Care Model. This position is responsible for assessing member needs, coordinating services and ensuring that patients receive timely, appropriate and person-centered care. This field-based role includes regular travel throughout the Capital Region, including Albany and surrounding counties. Remote work flexibility is available when not conducting field visits.

Candidates must reside in New York State.


Key Responsibilities

  • Conduct comprehensive patient assessments, including the New York State UAS assessment, in members’ homes at least twice annually and as required.

  • Develop individualized care strategies focused on the most appropriate and least restrictive level of care.

  • Coordinate referrals for community, psychosocial, state-funded and financial support services.

  • Serve as the primary care coordination contact throughout the continuum of care.

  • Communicate necessary health information to providers, stakeholders and interdisciplinary teams to ensure timely and coordinated care.

  • Advocate for member needs and patient preferences to ensure integrated, person-centered care delivery.

  • Maintain accurate documentation and participate in audits, training and ongoing quality improvement efforts.


Required Experience and Qualifications

  • Active, unrestricted Registered Nurse (RN) license in New York State.

  • Minimum 2 years of clinical experience in a healthcare setting.

  • Minimum 1 year of community-based case management experience, including coordinating care for individuals with complex medical, behavioral or social needs.

  • Experience in long-term care, home health, hospice, assisted living or public health.

  • Proficiency in Microsoft Outlook, Word and Excel.

  • Valid New York State ID (or ability to obtain prior to hire).

  • Residency in New York State.

  • Valid Driver’s License and reliable transportation.

  • Ability to travel across the Capital Region, visiting members in homes, community facilities, hospitals and provider locations.


Preferred Qualifications

  • Behavioral health or advanced clinical degree.

  • Experience with electronic charting systems.

  • Experience coordinating community-based resources.

  • Background in field-based care management.

  • Experience managing populations with significant medical or behavioral complexities.

  • Proficiency in UAS-NY.

Compensation and Benefits

  • Hourly Pay Range: USD 28.27 – 50.48 (based on full-time schedule, experience and market conditions).

  • Compensation may vary based on certifications, education and local labor factors.
