Job Schedule:

  • Monday-Friday: 7:00 AM - 3:00 PM, 10:30 AM - 6:30 PM, 11:30 AM - 7:30 PM (Rotating shifts)
  • Rotating Saturdays: 8:00 AM - 4:00 PM
  • Rotating Sundays: 8:00 AM - 2:00 PM

Key Responsibilities:

  • Evaluate Medication Orders: Review physician medication orders for appropriateness, dosage, interactions, and route of administration. Recommend therapy changes as necessary.
  • Maintain Patient Records: Update and manage patient/resident databases, including medical history, clinical and laboratory data, and concurrent medications.
  • Monitor Laboratory Tests: Ensure appropriate laboratory tests for high-risk conditions are ordered, evaluated, and communicated to the healthcare team.
  • Collaborate with Healthcare Team: Provide clinical and technical support to physicians, nurses, and other team members.
  • Document Communication: Maintain accurate documentation of interactions with healthcare providers, patients, and their families in the medical record.
  • Medication Safety: Implement policies for the safe procurement, storage, distribution, and disposal of drugs and biologicals.
  • Compliance: Ensure adherence to federal and state regulations and maintain required documentation.
  • Medication Errors: Report medication-related errors to the Pharmacy Manager.
  • Assist Staff: Provide guidance on safe and effective methods for drug procurement, administration, and disposal.
  • Additional Tasks: Perform other related tasks as assigned.

Qualifications:

  • Education: Bachelor’s Degree in Pharmacy required; PharmD preferred.
  • Experience: At least 1 year of experience in long-term care preferred.
  • Licenses/Certifications: As required by government regulations and business needs.
  • Skills: Attention to detail, goal-oriented, strong teamwork, and effective oral communication.
