Position Summary

As a Clinical Pharmacist I, you will ensure safe, effective, and cost-efficient medication management for patients. You will provide pharmacy services with compassion, excellence, and efficiency, supporting clinical teams and patients across inpatient and ambulatory care settings.


Key Responsibilities

Operational Duties

  • Prepare and dispense medications per physician orders, following established policies, procedures, and protocols.

  • Interpret and verify drug orders accurately in computerized patient medication records.

  • Compound and dispense non-sterile and sterile pharmaceuticals, including chemotherapy and parenteral nutrition products.

  • Manage controlled substances and maintain accurate records in compliance with legal and institutional requirements.

  • Supervise pharmacy technicians and verify daily activities to ensure operational accuracy.

  • Maintain current pharmacist licensure and fulfill continuing education requirements.

Ambulatory Care Duties

  • Provide pharmaceutical care under approved protocols or collaborative agreements.

  • Manage patient drug therapy, anticoagulation management, patient education, and medication preparation.

  • Participate in clinical programs such as transition of care, discharge medication reconciliation, and intensive medical home services.

Clinical Duties

  • Monitor and optimize patient drug therapy for safety, efficacy, and cost-effectiveness.

  • Detect and report adverse drug reactions promptly.

  • Provide clinical consultation, drug information, and patient education to medical staff and patients’ families.

  • Support pharmacokinetics, renal dosing, antibiotic stewardship, opioid stewardship, and medication shortage management.

  • Participate in interdisciplinary rounds and quality improvement initiatives.


Occupational Hazards

  • Potential exposure to hazardous drugs, cytotoxic agents, biologic materials, and sharp instruments.

  • Risk of musculoskeletal injury during lifting or patient care activities.

  • Adherence to safety protocols is required to minimize exposure risks.


Qualifications & Requirements

Education:

  • Doctor of Pharmacy (PharmD) or Bachelor of Pharmacy (BS Pharmacy)

Experience:

  • Hospital pharmacy experience preferred

Licensure & Certifications:

  • State pharmacy licensure required within 60 days of start date

  • BLS or ACLS required

  • PALS required for pediatric practice

Competencies:

  • Strong clinical knowledge and attention to detail

  • Ability to work collaboratively in a fast-paced hospital environment

  • Excellent communication and organizational skills

Work Schedule:
