Role Overview:
UCLA Health is seeking a Clinical Oncology Pharmacist to join its Ambulatory Infusion Services team. In this vital role, you will provide expert clinical pharmacy services to oncology patients, ensuring safe, accurate, and effective medication therapy. You will review and verify physician orders, maintain patient medication profiles, and collaborate closely with healthcare teams to optimize patient outcomes.

Key Responsibilities:

  • Maintain and review patient medication profiles for oncology infusion patients.

  • Verify and check physician medication orders, including IV products, for accuracy and safety.

  • Provide clinical pharmacy services such as pharmacokinetic dosing, IV/PO conversions, therapeutic interchange, and pharmacist-led interventions.

  • Collaborate with physicians, nurses, and other healthcare professionals to promote safe medication use and prevent inappropriate or unsafe therapies.

  • Ensure adherence to established clinical protocols and regulatory standards.

  • Participate in quality improvement initiatives and contribute to process enhancements within the pharmacy department.

  • Support patient education regarding medications, side effects, and therapy management.

Qualifications & Experience:

  • Doctor of Pharmacy (Pharm.D.) or equivalent degree in Pharmacy.

  • Valid pharmacist licensure in California.

  • Clinical experience in oncology pharmacy or infusion services preferred.

  • Strong knowledge of IV compounding, chemotherapy protocols, and pharmacokinetics.

  • Excellent clinical judgment, problem-solving, and communication skills.

  • Ability to work variable schedules, including weekends and holidays.
