Job Schedule:

  • Monday-Friday:
    • 7:00 AM - 3:00 PM
    • 10:30 AM - 6:30 PM
    • 11:30 AM - 7:30 PM (Rotating shifts)
  • Rotating Weekends:
    • Saturday: 8:00 AM - 4:00 PM
    • Sunday: 8:00 AM - 2:00 PM

Key Responsibilities:

  • Medication Order Evaluation: Review physician medication orders for appropriateness, dosage, potential interactions, and route of administration. Recommend therapy changes to physicians and facility staff as necessary.
  • Patient/Resident Database Management: Maintain accurate records of current and past medical histories, clinical and laboratory data, and concurrent medications (including over-the-counter and home remedies).
  • Laboratory Test Monitoring: Ensure appropriate laboratory tests are ordered and discussed with healthcare team members to monitor high-risk conditions.
  • Collaboration with Healthcare Team: Work closely with healthcare providers to offer clinical and technical support.
  • Documentation: Document all communication with physicians, nurses, other healthcare team members, patients, and families in the medical record.
  • Medication Safety: Implement policies for the safe procurement, storage, distribution, and disposal of drugs and biologicals.
  • Regulatory Compliance: Ensure compliance with federal and state regulations and maintain all required documentation.
  • Error Reporting: Inform the Pharmacy Manager of any medication-related errors.
  • Staff Assistance: Assist staff with safe procedures for drug procurement, administration, and disposal.
  • Other Duties: Perform additional tasks as assigned.

Qualifications:

  • Education: Bachelor’s Degree in Pharmacy required; PharmD preferred.
  • Experience: At least 1 year of experience in long-term care preferred.
  • Licenses/Certifications: As required by government regulations and business needs.

    • Ability to work under pressure and handle critical situations
