Position Summary:

The Population Health Coordinator supports SGMG’s quality management goals related to improving the health of the SGMG patient population by utilizing advanced skills in data analytics, reporting, and developing tools to make data actionable. Under the supervision of SGMG Administration, this role provides ongoing support, facilitation, and coordination between new and established physicians and staff for population health activities. Responsibilities include communicating with patients and their insurance providers, scheduling patients, performing clerical and clinical applications, and submitting data for various quality measure incentive programs. The incumbent will also participate in quality improvement projects and collaborate with clinical and operational teams to support the quality teams in achieving their goals and objectives.

Minimum Qualifications:

Education:

  • High school graduate, minimum; certification from an approved school with a certificate in Medical Assisting preferred.
  • Medical terminology required.
  • Ability to perform computer entry and basic office computer skills.

Required Length and Type of Experience:

  • Externship of at least 100 hours or 3 years of Medical Assistant experience in an outpatient setting.

Required Licensure, Certification, or Registry:

  • Medical Assisting certificate preferred.
