About the Role

The Field Case Manager plays a central role in this mission by managing a panel of members with complex behavioral health needs, including mental health conditions and substance use disorders.

This position supports an Integrated Care Model and focuses on coordinating medical, behavioral, and social services for members. The role requires regular fieldwork within Hillsborough County, including home visits and visits to long-term care facilities.


Key Responsibilities

  • Conduct comprehensive assessments and develop individualized care plans aligned with the most appropriate level of care.

  • Coordinate referrals to social service and community programs, including financial assistance, psychosocial support, and state-supported resources.

  • Serve as the primary point of contact for members throughout the care continuum.

  • Communicate essential health information to all care stakeholders to ensure timely and coordinated care delivery.

  • Advocate for members to ensure their preferences, needs, and rights are accurately represented in care planning.

  • Spend approximately 80% of work hours in the field meeting members in homes or long-term care settings.

  • Support team members by providing behavioral health insights and case-related information.


Required Experience and Qualifications

  • Bachelor’s degree with a minimum of 2 years of relevant experience, or 6+ years of experience in social services or case management without a degree.

  • Minimum 1 year of experience with MS Office (Word, Excel, Outlook).

  • Experience in long-term care, home health, hospice, public health, or assisted living.

  • Familiarity with local behavioral health providers and community support programs addressing social determinants of health (SDOH).

  • Reliable transportation with the ability to travel across Hillsborough County.

  • Bilingual proficiency in Spanish (mandatory).


Preferred Qualifications

  • Prior field-based case management experience.

  • Experience in long-term care (LTC) case management.

  • Knowledge of electronic health charting systems.

  • Experience arranging community-based resources and referrals.

  • Background in supporting individuals with co-occurring behavioral health and substance use disorders.

  • Experience working with medically and behaviorally complex populations.


Work Environment and Compensation

  • Remote work available for eligible candidates under the Telecommuter Policy.

  • Compensation range: $23.41 – $41.83 per hour (full-time), based on location, education, experience, and certifications.
