Position Overview

Optum, a global healthcare organization, is dedicated to delivering technology-enabled care and improving patient health outcomes. As a Field Based Community Health Worker, you will work directly with Medicaid members to support care coordination, conduct home-based and community assessments, and address barriers related to social determinants of health such as transportation, housing, and food access.

This role combines patient engagement, community outreach, and care management to ensure members receive timely, appropriate, and coordinated healthcare services. You will serve as a single point of contact for each assigned member while supporting their transition across care settings and encouraging regular follow-up after hospitalization.


Key Responsibilities

  • Conduct assessments, create individualized care plans, and assist members in accessing the least restrictive and most appropriate level of care.

  • Utilize company and community resources to develop effective case management strategies.

  • Perform home visits, community visits, and telephonic outreach to engage members.

  • Identify and coordinate referrals for financial assistance, psychosocial services, and state/community programs.

  • Communicate essential health information to care teams, family members, and stakeholders.

  • Advocate for members’ needs, ensuring their preferences are represented in the healthcare plan.

  • Document assessments, care plans, referrals, and progress in electronic health records.

  • Support utilization management benchmarks and quality outcomes.

  • Assist members with setting SMART goals and follow up on progress.

  • Conduct post-discharge outreach and facilitate connection to social service agencies.

  • Adhere to detailed documentation standards and follow motivational interviewing practices.

  • Manage multiple tasks, maintain accuracy, and work independently within deadlines.


Work Schedule & Travel Requirements

  • Schedule: Monday to Friday, 8:00 AM to 5:00 PM CST

  • Work Setting: Field-based role with a home-based office

  • Travel: Up to 50% local travel within Houston and surrounding Harris County (mileage reimbursed at government rate)


Required Qualifications (Experience Mandatory)

  • High School Diploma/GED or higher

  • Minimum 2 years of field-based experience

  • Minimum 1 year of experience with community resources, culture, and local support systems

  • Minimum 1 year of experience using MS Word, Excel, and Outlook

  • Completion of State-approved Community Health Worker (CHW) course

  • Residency within a commutable distance to Houston and Harris County

  • A quiet, secure home workspace for telecommuting

  • Reliable transportation and the ability to travel up to 100 miles round trip

  • Valid, unrestricted Texas Driver’s License and state-required insurance


Preferred Qualifications

  • Licensed Practical Nurse (LPN), Certified Nursing Assistant/Home Health Aide, or Certified Medical Assistant

  • Experience supporting populations with complex medical and behavioral conditions

  • Experience with electronic health charting systems

  • Background in arranging community resources and support services

  • Knowledge of Medicaid populations and community health programs

Compensation and Benefits

  • Hourly pay range: $20.00 to $35.72 (based on full-time employment, dependent on education, experience, certifications, and local market conditions)
