Job Opportunity: Pharmacist Float Pool at St. Elizabeth Healthcare

Location: Acute Care Facilities (Kentucky and Indiana)
Job Type: Regular, Full-Time (35 hours per week)
Schedule: 7 On / 7 Off (Third Shift)


Position Summary

Join the Pharmacist Float Pool at St. Elizabeth Healthcare, a unique opportunity to support acute care facilities across the region. This role offers premium pay and bonuses, with the flexibility of a 7 days on, 7 days off schedule. You’ll provide pharmaceutical care with an emphasis on clinical monitoring and therapeutic interventions to improve patient outcomes. The Pharmacist Float Pool will float to various facilities and offer support in all shifts, ensuring timely medication distribution, safety, and accurate clinical decision-making.


Key Responsibilities

  • Medication Order Verification:

    • Review and interpret medication orders from licensed prescribers, ensuring appropriateness for the patient's condition.
    • Assess drug interactions, allergy alerts, and therapeutic outcomes, recommending alternatives when necessary.
    • Perform second verification for pediatric and chemotherapy orders.
  • Therapeutic Clinical Monitoring:

    • Monitor ongoing medication therapy compliance with Collaborative Care Agreements and medication management protocols.
    • Contact prescribers with therapeutic alternatives when needed and document assessments in patient charts.
  • Compounding and Dispensing Pharmaceuticals:

    • Ensure accurate and timely dispensing of medications, including compounding according to USP standards.
    • Supervise medication repackaging and distribution via automated systems and Pyxis systems.
  • Supervision and Mentorship:

    • Supervise and mentor Pharmacy Interns and Technicians, ensuring tasks are completed on time and according to ICARE values.
  • Medication Use Safety Initiatives:

    • Report medication variances and identify opportunities for improvement in medication safety practices.
    • Participate in adverse drug reaction reporting.
  • Controlled Substance Accountability:

    • Participate in and supervise controlled substance receipt, dispensing, and distribution, ensuring proper reconciliation of inventories.
  • Contributing to Financial Goals:

    • Encourage formulary adherence and provide therapeutic alternatives for non-formulary requests.
    • Participate in IV to PO conversions and minimize overtime.
  • Staff Development and Education:

    • Provide drug information and educational programs to medical staff, nursing staff, and patients.
    • Serve as a preceptor to Pharmacy Residents and Interns.

Qualifications

  • Education: Graduate from an ACPE-accredited pharmacy school.
  • Licensure:
    • Current pharmacy license (or eligible) in Kentucky and Indiana (Indiana licensure required within 90 days if licensed in Kentucky).
  • Experience: At least 6 months of experience as a pharmacist in a hospital or comparable setting.
  • Specialized Knowledge:
    • Strong problem-solving and critical thinking skills.
    • Familiarity with current pharmacy practice standards (ASHP).
    • Effective communication skills.
