Summary:

The Clinical Pharmacist provides direct patient-centered care and integrated pharmacy services in a decentralized setting. Responsibilities include medication therapy management, disease-specific pharmaceutical care, and centralized support across health center teams.


Responsibilities:

  • Deliver ambulatory outpatient pharmacy services per national/local guidelines and facility policies.

  • Design, implement, and assess medication treatment plans that are safe, effective, and cost-efficient.

  • Monitor treatment outcomes and modify plans based on assessments.

  • Take patient histories, evaluate medications, and perform screenings (e.g., depression, tobacco, alcohol).

  • Maintain EHR records of clinical activities, allergies, and screenings.

  • Order and interpret lab results relevant to medication monitoring.

  • Offer comprehensive medication management for chronic disease patients.

  • Discontinue outdated or duplicate medications.

  • Collaborate with providers and administration to coordinate care.

  • Participate in Pharmacy & Therapeutic Committee and handle Prior Authorization requests.

  • Supervise or train students as needed.

  • Perform staff pharmacist duties and contribute to reporting and team goals.


Skills Required:

  • Independent decision-making and judgment.

  • Knowledge of patient care charts and drug regulations.

  • Strong interpersonal and team communication skills.

  • Understanding of infection control, accreditation, and certification requirements.

  • Ability to train and supervise staff.

  • Proficient with Microsoft Office (Word, Excel, Access, PowerPoint).

  • Perform basic patient assessments and data analysis.

  • Fluent in English—written and verbal.


Education Requirements:

Essential:

  • Doctor of Pharmacy (PharmD)

Accepted Equivalent:

  • Bachelor of Science in Pharmacy (B.S.Pharm)


Credentials:

Essential:

  • Licensed as a Registered Pharmacist by the Wisconsin Department of Registration and Licensing

  • Must pursue board certification when eligible

Preferred (Nonessential):

  • Basic Life Support (BLS) certification (within 90 days)

  • Oneida Certification in Child Abuse and Neglect Reporting (within 90 days)

  • HIPAA training (prior to start date)


Experience:

Essential:

  • At least 1 year of pharmacy experience in hospital, clinic, or retail setting

Preferred:

  • Experience with Robotic Dispensing Systems


Working Conditions:

Physical:

  • Frequent walking, sitting, talking, hearing, reaching

  • Occasional standing, bending, kneeling, crouching, crawling

  • Able to lift and move up to 25 pounds

Environment:

  • Moderate noise in a climate-controlled setting

  • May require irregular shifts, extended hours, evenings, or weekends

Health & Safety:
