UnitedHealthcare is seeking a Bilingual RN Field Clinical Care Coordinator to support its Integrated Care Model across Boston, MA, and nearby communities. This role combines direct field outreach with telehealth coordination to ensure members receive comprehensive, equitable, and high-quality care.

Candidates residing in or near Allston, Back Bay, Brighton, Fenway, Jamaica Plain, Mission Hill, Kenmore, Roxbury, West Roxbury, South End, Beacon Hill, Charlestown, Dorchester, East Boston, North End, South Boston, South Cove, West End, Roslindale, Hyde Park, Mattapan are preferred. Remote work flexibility is available under UnitedHealth Group’s Telecommuter Policy.


Role Overview

As a Field Clinical Care Coordinator, you serve as the primary contact for member assessments, care planning, coordination with providers, and ensuring members access the right level of care. This position involves approximately 50% field travel to member homes or long-term care facilities.


Key Responsibilities

  • Conduct comprehensive face-to-face and telephonic assessments covering medical, functional, behavioral, cultural, and social determinants of health.

  • Develop and implement individualized care plans aligned with the least restrictive and most appropriate care setting.

  • Coordinate referrals to social, financial, psychosocial, and community support programs.

  • Manage member care plans across the continuum as a single point of contact.

  • Collaborate with internal care teams, healthcare providers, and community partners.

  • Communicate essential health-related information to ensure efficient, high-quality care delivery.


Required Qualifications

  • Active and unrestricted Registered Nurse (RN) license in Massachusetts.

  • Minimum 1 year of clinical experience (hospital, community, home health, or related).

  • Proficiency in Microsoft Word, Excel, and Outlook.

  • Ability to work from a secure, private home workspace with PHI protection.

  • Must reside in an area with approved high-speed internet access.

  • Reliable transportation and a valid U.S. driver’s license for field visits.

  • Bilingual proficiency in Cantonese or Mandarin.


Preferred Qualifications

  • Bachelor’s or Master’s Degree in Nursing.

  • Certified Care Manager (CCM) credential.

  • 1+ year of experience in community case management for individuals with complex care needs.

  • Experience in team-based care and/or managed care settings.

  • Familiarity with Electronic Medical Records (EMR) and digital documentation workflows.


Physical Requirements

  • Ability to move between office and field locations multiple times daily.

  • Ability to travel across varied terrain for member visits.

  • Ability to transport work equipment such as laptops and documentation devices.

  • Ability to sit for extended periods for computer-based tasks.


Compensation and Benefits

Compensation is based on experience, certifications, and market factors.
Hourly Range: $28.27 – $50.48 (for full-time employment)
