Job Summary

WellMed, part of Optum, is hiring a licensed Social Worker in Austin, Texas to deliver end‑to‑end assessment, care planning, care coordination, and psychosocial support for patients with complex, catastrophic, behavioral health and SDOH (Social Determinants of Health) needs. You will ensure compliance with Medicare/CMS, state and federal requirements, collaborate closely with physicians and interdisciplinary teams, and champion patient advocacy and health equity across the continuum of care.


Responsibilities

  • Perform comprehensive biopsychosocial assessments and document to CMS standards.

  • Conduct patient assessments in multiple settings: home, PCP/specialist clinics, hospitals, SNFs.

  • Develop, implement, and monitor individualized care plans focused on SDOH and psychosocial barriers.

  • Coordinate federal, state, local, and community resources within plan eligibility to close care gaps.

  • Serve as a liaison/advocate among patients, families, providers, and external agencies.

  • Participate in Interdisciplinary Team (IDT) meetings and Performance Improvement (PI) initiatives.

  • Maintain licensure, certifications, and annual trainings; adhere to WellMed core values and policies.

  • Remain current on health plan benefits, Medicare guidelines, and managed care processes.

  • Perform other duties as assigned.


Required Skills & Qualifications

  • Bachelor of Social Work plus 3+ years of social work experience in healthcare.

  • Texas license required: LBSW, LMSW, or LCSW.

  • 3+ years of experience in a health care environment (managed care, disease management, or care coordination preferred).

  • Proficient in MS Office (Word, Excel, Outlook, Notes) and departmental applications.

  • Strong written/verbal communication, prioritization, organization, and multitasking skills.

  • Ability to work independently and within IDTs, with a commitment to health equity and patient-centered care.

Preferred Qualifications

  • MSW with active licensure (Texas).

  • Experience with geriatric, behavioral/mental health, and long-term care populations.

  • Knowledge of managed care, medical terminology, referral processes, claims, CPT/ICD coding.

  • Bilingual (English/Spanish) proficiency.
