Job Summary:
The Pharmacist at The Jewish Hospital is responsible for performing medication management activities to ensure safe, rational, and cost-effective therapy. This includes compliance with all applicable rules, regulations, and standards from federal, state, and accrediting agencies. The Pharmacist will adhere to departmental policies and objectives, contributing to high standards in patient care.

Essential Functions:

  • Process medication orders and provide medication reviews for appropriateness, safety, and efficacy.
  • Ensure accurate preparation of compounded and non-compounded products following safety and regulatory guidelines.
  • Oversee the distributive functions of the pharmacy department, ensuring timely medication availability to patients and staff.
  • Supervise pharmacy technicians and support personnel as needed.
  • Maintain security and accountability of pharmacy inventory.
  • Identify, document, and resolve potential and actual adverse medication events, using the event reporting system for trending and prevention.
  • Provide medication education to enhance patient understanding and adherence.
  • Collaborate with the pharmacy team, healthcare providers, patients, and caregivers to communicate effectively.
  • Participate in departmental activities such as medication safety, quality assurance, drug utilization reviews, drug dosing, and student precepting.

Education Requirements:

  • Graduate of an ACPE accredited School of Pharmacy with a BS or PharmD (required).
  • Advanced pharmacy degree (Doctor of Pharmacy, Masters, Residency) is preferred.

Required Licensing & Certifications:

  • Current pharmacist license in the state(s) where practicing (required).
  • Valid state-issued ID (required).
  • BLS Basic Life Support (preferred).
  • Immunization Certified (preferred).

Experience Requirements:

  • Minimum of 2 years of recent experience in an acute care setting (required).
  • 1 year of leadership experience (preferred).
  • Experience in pharmacy practice (required).

Skills & Abilities:

  • Ability to monitor patient conditions, administer medications, and communicate with medical professionals.
  • Proficient in analyzing data, budgeting, and scheduling.
  • Critical thinking, attention to detail, and strong communication skills.
  • Experience with pharmacy operating systems and automation.
  • Knowledge of medications, including adverse effects, interactions, dosing, and monitoring.
  • Regulatory compliance expertise and ability to develop SMART goals.
  • Ability to provide medication education and coach team members.
  • Strong organizational and stress management skills.

Training:

  • EPIC Electronic Health Record (preferred).
  • Proficient use of computer hardware and software (required).
