Pharmacist – Float Pool (Third Shift)
Location: Uppsala (Hybrid)
Job Type: Regular, Full-Time
Scheduled Hours: 35 Hours/Week

Job Summary:

This position is part of the third-shift float pool, following a 7 On/7 Off schedule, and provides support across Acute Care Facilities. The Pharmacist Float Pool will float to various SEH facilities to offer pharmacy services as per staffing needs, including all shifts and acute care sites.

You will play a crucial role in delivering Pharmaceutical Care with an emphasis on clinical monitoring and therapeutic interventions that enhance patient outcomes. Additionally, you will provide educational programs, support Performance Improvement processes, and oversee the timely and accurate dispensing and distribution of medications.

While upholding the ICARE values and exhibiting the AIDET principles, you will demonstrate respect, dignity, kindness, and empathy in every encounter with patients, families, visitors, and staff, regardless of cultural background.

Job Description:

Medication Order Verification:

  • Review and interpret medication orders from licensed prescribers to ensure they meet the necessary requirements.
  • Assess medication appropriateness based on the patient’s condition and look for potential issues like drug interactions or allergies.
  • Consult with the prescriber for clarification if the order is incomplete or may result in suboptimal therapeutic outcomes.
  • Provide therapeutic alternatives when necessary.
  • Perform second verification of pediatric and chemotherapy orders.

Therapeutic Clinical Monitoring:

  • Actively monitor medication therapy as per Collaborative Care Agreements and medication management protocols.
  • Provide therapeutic recommendations and collaborate with prescribers to optimize patient care.
  • Document medication therapy assessments and recommendations in the patient’s chart, sharing information with the interdisciplinary team.

Compounding and Dispensing Pharmaceuticals:

  • Ensure accurate, timely dispensing of the correct medications.
  • Supervise the compounding process, ensuring USP standards, ingredient compatibility, content verification, and proper labeling.
  • Use barcode technology when applicable to ensure accurate dispensing.
  • Supervise and verify the repackaging of medications, ensuring correct documentation.

Supervision and Staff Development:

  • Supervise and mentor Pharmacy Interns and Technicians, ensuring timely completion of tasks and providing guidance on ICARE values and the AIDET model.
  • Report non-responsive behaviors to leadership.

Medication Use Safety Initiatives:

  • Detect and report medication variances promptly.
  • Participate in efforts to increase the safe use of medications through initiatives like barcode availability and Epic improvements.
  • Report suspected adverse drug reactions as needed.

Controlled Substance Accountability:

  • Participate in the receipt, dispensing, and distribution of controlled substances, ensuring proper inventory management and reconciliation.
  • Assist in resolving discrepancies related to controlled substances.

Financial Contribution:

  • Encourage adherence to formulary restrictions and suggest appropriate therapeutic alternatives for non-formulary requests.
  • Participate in IV to PO conversions and optimize resource usage to reduce overtime.

Drug Information and Staff Development:

  • Provide accurate drug information to medical and nursing staff, as well as patients.
  • Serve as a preceptor or provide advice to Pharmacy Residents and Interns.
  • Contribute to staff education programs.

Professional Competency:

  • Maintain licensure and complete required competency assessments.
  • Stay current on pharmacy practices and seek specialized knowledge development.

Education, Credentials, and Licenses:

  • Graduate of an ACPE-accredited pharmacy college.
  • Licensed (or eligible) in Kentucky and Indiana. If licensed in Kentucky, Indiana licensure must be obtained within 90 days of the start date.

Specialized Knowledge:

  • Strong problem-solving and critical thinking skills.
  • Effective communication abilities.
  • Familiarity with current practice standards (ASHP).

Experience:

  • At least 6 months of pharmacist experience in a hospital or similar practice setting.
