Pharmacist (Per Diem/Relief, Day Shift) – Florence, Oregon

Hourly Compensation: $64.78 - $97.18 (based on qualifications, professional experience, and/or work location).

Position Details:

  • Review medication orders for appropriateness, including contraindications, allergies, dose, route, frequency, duplicate therapy, and formulary status.
  • Interpret, clarify, and rectify physician medication/IV orders and prescriptions, including medication reconciliation.
  • Monitor patients on target drugs and initiate pharmacokinetic dosing protocols.
  • Evaluate drug therapy for appropriateness and recommend adjustments to clinicians.
  • Prescribe drug therapy per collaborative agreements or approved protocols.
  • Dispense medications and provide patient counseling.
  • Detect, monitor, and report adverse drug events and medication errors.
  • Supervise and train pharmacy technicians, interns, and externs; may serve as a preceptor.
  • Participate in the development of drug protocols, clinical pathways, and drug usage evaluations.
  • Complete quality assurance audits as required.

Qualifications:

  • Graduate from an accredited School of Pharmacy with a BS or PharmD degree.
  • Basic clinical pharmacy skills and customer service abilities.
  • Current or eligible Registered Pharmacist license for the state of Oregon.
  • Proficiency in computer applications and basic mathematical/analytical skills.
  • Strong planning, organizational, and communication skills.
  • Ability to work independently and manage priorities effectively.

Florence, Oregon-Specific Requirements:

  • Basic Life Support (BLS) certification within six months of hire.
