Job Title: Outpatient Coder IV (Remote)


Job Overview

The Outpatient Coder IV is responsible for accurately abstracting and coding medical data in compliance with ICD-10-CM, CPT, and HCPCS guidelines, as well as CMS directives. This role involves data entry into electronic medical records, physician queries, and ensuring compliance with reimbursement and regulatory requirements.


Key Responsibilities

  • Assign codes for reimbursements, research, and regulatory compliance, following established guidelines and conventions.
  • Code and abstract patient encounters, including diagnostic and procedural information, reportable elements, and complications.
  • Analyze medical records to identify documentation deficiencies and ensure accuracy.
  • Verify that documentation supports diagnoses, procedures, and other charges.
  • Identify reportable elements, complications, and quality measures.
  • Communicate with physicians to clarify documentation through the query process.
  • Assign CPT, HCPCS, and ICD-10-CM codes with precision.
  • Address NCCI edits and NCD/LCD coverage determinations as needed.
  • Maintain required productivity and quality benchmarks.
  • Adhere to organizational standards and behavioral competencies.
  • Perform additional duties and projects as assigned.

Qualifications

Education and Experience:

  • High school diploma or equivalent (GED).
  • Minimum of 3+ years of coding experience in a Trauma Level 1 and Academic Teaching Facility setting.
  • Strong knowledge of medical terminology, physiology, and anatomy.

Skills:

  • Proficiency in coding for the following areas:
    • Ancillary Accounts (e.g., Diagnostic Radiology, Cardiology).
    • Emergency Department and Infusion Services (e.g., Oncology).
    • Observation and Procedure Room (e.g., Endoscopies, Cardiac Cath).
    • Outpatient Services, particularly Operating Room procedures.
  • Strong computer skills, including Microsoft Office and Google Suite.
  • High typing speed with accuracy.
  • Excellent written and verbal communication skills.

Licenses and Certifications:

  • Certification through at least one of the following organizations:
    • American Health Information Management Association (AHIMA)
    • American Academy of Professional Coders (AAPC)
