Job Summary

The Coder ER reviews medical records and performs accurate coding of diagnoses, procedures, DRG/APC, and charge codes under limited supervision. The role ensures compliance with coding guidelines and supports reimbursement, research, quality evaluation, and patient care. Confidentiality of patient records must be maintained at all times.


Key Responsibilities

  • Abstract, code, sequence, and interpret clinical information from inpatient, outpatient, ED, pro fee, and clinical records.

  • Assign correct principal and secondary diagnoses and procedures, ensuring accurate sequencing.

  • Use technical coding principles and DRG/APC reimbursement expertise.

  • Abstract and code data into multiple software programs/encoders.

  • Follow official coding guidelines for analysis and reporting.

  • Maintain compliance with regulatory, accreditation, State, and Federal requirements.

  • Enter codes for reporting medical services, including radiology, injections, infusion, biopsies, and physician services.

  • Track and document productivity in designated systems; maintain 95% quality rating.

  • Perform duties in compliance with company policies including HIPAA.

Work Environment

  • Remote home office.

  • Routine use of standard office equipment (computers, phones).


Physical Demands

  • Occasionally required to stand, walk, sit, use hands, reach, climb stairs, balance, stoop, kneel, crouch, or talk/hear.

  • Occasionally lift/move up to 25 pounds.

  • Specific vision abilities: close, distance, peripheral, depth perception, and focus adjustment.


Education and Experience

  • Required: Completion of AAPC or AHIMA-approved Coding Certificate Program.

  • Minimum 2–4 years of recent production coding experience in acute care and professional fee (pro fee).

  • Preferred: N/A


Certifications

  • CPC, COC, CIC, RHIA, RHIT, CCS, and/or CCS-P.


Technical Requirements & Skills

  • Facility ED coding experience in Trauma Level 1 and Teaching hospitals.

  • ICD-10-CM and CPT coding proficiency.

  • Facility ED level assignment and professional EM billing.

  • Injection and infusion coding experience.

  • Experience with Epic and 3M 360 CAC.

  • Maintain minimum 95% coding accuracy.

  • Ability to meet productivity expectations.

  • Strong communication skills, self-motivated, disciplined, and team-oriented.

  • Ability to quickly learn new technical workflows.

  • Successful completion of a written assessment.
