Description:
Join Option Care Health, the largest independent home and alternate site infusion services provider in the U.S., as a Clinical Pharmacist. In this full-time role, you will be responsible for providing safe, effective, and appropriate pharmacy services to Option Care patients, including those receiving home infusion care. You will coordinate patient services, monitor treatment progress, and educate patients, caregivers, and healthcare staff about medication use.

As a Clinical Pharmacist, you will play a critical role in optimizing medication therapy, ensuring the continuity of care between home and healthcare settings, and adhering to regulatory standards. Your expertise will contribute to enhancing patient outcomes and safety, supporting Option Care’s commitment to compassionate care.


Key Responsibilities:

  • Assess patients' suitability for home care based on medical conditions, prescribed medication therapies, and outcome goals.
  • Design, monitor, and evaluate individualized pharmacy care plans for home care patients, adjusting therapy as needed.
  • Provide education and counseling to patients, caregivers, and staff on medication use and safety.
  • Ensure medication is dispensed accurately, following policies, regulatory requirements, and safety guidelines.
  • Manage medication administration equipment, ensuring proper maintenance and troubleshooting.
  • Respond to drug information requests and stay up-to-date with medical literature to support informed decision-making.
  • Ensure continuity of care across various settings, maintaining accurate patient information and collaborating with healthcare teams.
  • Participate in quality assurance processes and report any medication-related incidents or discrepancies.

Qualifications:

  • Education: Graduate from an accredited School of Pharmacy.
  • Licensure: Active pharmacy license in the state of employment.
  • Experience: At least 1 year of experience in sterile IV admixture procedures, non-sterile compounding, immunizations, or medication therapy management (MTM), or completion of an ASHP-accredited Pharmacy Residency Program.
  • Skills: Proficiency in Microsoft Word, Excel, and PowerPoint. Strong customer service, organizational, and communication skills.
  • Travel Requirements: Willingness to travel up to 5% of the time for business purposes.
