About the Role:
Optum Home & Community Care, a part of UnitedHealth Group, is seeking an experienced Care Coordinator to support care transitions for patients in Skilled Nursing Facilities (SNFs) across Lexington, KY. In this hybrid role, you will be the critical link between patients, families, and healthcare teams—ensuring efficient discharge planning and better recovery outcomes for older adults transitioning from hospital to home.

This position is part of the naviHealth product line, which supports innovative post-acute care models that prioritize patient independence and seamless coordination.


Key Responsibilities:

  • Conduct weekly SNF assessments using CMS criteria to monitor patients' progress and appropriateness of care.

  • Facilitate discharge planning with physicians, social workers, physical therapists, and families.

  • Coordinate authorizations, concurrent reviews, and referrals to appropriate health plans and high-risk case managers.

  • Provide timely and accurate documentation in the Coordinate platform.

  • Attend weekly care rounds and interdisciplinary meetings to resolve barriers to discharge.

  • Educate patients and families and participate in care conferences as needed.

  • Monitor metrics and dashboards to improve patient outcomes and reduce readmissions.

  • Support startup of new contracts and maintain HIPAA compliance across operations.


Required Qualifications:

  • Active, unrestricted clinical license in Kentucky as RN, PT, OT, or Speech-Language Pathologist

  • Minimum 5 years of clinical experience

  • Reside within or near Lexington, KY

  • Valid Driver’s License and access to reliable transportation

  • Ability to travel locally to on-site facilities (within a 30-mile radius)


Preferred Qualifications:

  • Experience in geriatric care and transitions of care

  • Knowledge of utilization management, care coordination, or disease management programs

  • Strong background in home health, rehabilitation, or patient education

  • Skilled in Microsoft Office Suite

  • Demonstrated skills in communication, conflict resolution, and time management


Work Conditions:

  • Ability to set up a home office workspace

  • Capable of traveling up to 85% of the time within the local area

  • Able to work on-site and utilize laptops and mobile communication devices efficiently

  • Must comply with UnitedHealth Group’s drug-free workplace policy
