Nurse Practitioner – Dementia Care Specialist (Los Angeles, CA)
Job Summary:
UCLA Health is seeking a compassionate and experienced Nurse Practitioner – Dementia Care Specialist to join its interdisciplinary geriatrics care team in Los Angeles, CA. This full-time, on-site role supports dementia-focused patient care and education, working closely with families, physicians, and community clinics. You will play a pivotal role in implementing standardized procedures, modifying care regimens, and ensuring seamless, patient-centered care for elderly populations.
Salary Range:
NP2: $89.06 – $127.19/hour
NP1: $80.34/hour
Key Responsibilities:
Implement and manage care plans using Standardized Procedures (SPs)
Modify treatment regimens based on patient responses and observations
Provide and coordinate dementia and geriatrics-focused care
Educate patients and families on disease management and resources
Participate in interdisciplinary clinical rounds and team-based care
Conduct referrals and ensure continuity of care
Maintain detailed documentation of care provided
Support community clinics in West Los Angeles as needed
Required Skills & Qualifications:
Current California Registered Nurse (RN) and Nurse Practitioner (NP) license
Graduation from an accredited Nurse Practitioner program
National NP board certification (AGNP, FNP, or GNP preferred)
Prior experience in geriatrics, dementia, or neurology preferred (2–4 years recommended)
Ability to independently evaluate patients and adjust treatment plans
Knowledge of California Nursing Practice Act and relevant SPs
Excellent interpersonal, clinical judgment, and documentation skills
