Nurse Practitioner – Dementia Care Specialist (Los Angeles, CA)

Job Summary:
UCLA Health is seeking a compassionate and experienced Nurse Practitioner – Dementia Care Specialist to join its interdisciplinary geriatrics care team in Los Angeles, CA. This full-time, on-site role supports dementia-focused patient care and education, working closely with families, physicians, and community clinics. You will play a pivotal role in implementing standardized procedures, modifying care regimens, and ensuring seamless, patient-centered care for elderly populations.

Salary Range:

  • NP2: $89.06 – $127.19/hour

  • NP1: $80.34/hour


Key Responsibilities:

  • Implement and manage care plans using Standardized Procedures (SPs)

  • Modify treatment regimens based on patient responses and observations

  • Provide and coordinate dementia and geriatrics-focused care

  • Educate patients and families on disease management and resources

  • Participate in interdisciplinary clinical rounds and team-based care

  • Conduct referrals and ensure continuity of care

  • Maintain detailed documentation of care provided

  • Support community clinics in West Los Angeles as needed


Required Skills & Qualifications:

  • Current California Registered Nurse (RN) and Nurse Practitioner (NP) license

  • Graduation from an accredited Nurse Practitioner program

  • National NP board certification (AGNP, FNP, or GNP preferred)

  • Prior experience in geriatrics, dementia, or neurology preferred (2–4 years recommended)

  • Ability to independently evaluate patients and adjust treatment plans

  • Knowledge of California Nursing Practice Act and relevant SPs

  • Excellent interpersonal, clinical judgment, and documentation skills
