About the Role:
Join Optum Home & Community Care, part of UnitedHealth Group, as a Care Coordinator – Onsite to drive seamless transitions of care and improved outcomes for patients navigating post-acute settings. In this hybrid role based in Mayfield, KY, you’ll provide hands-on care coordination and assessments in Skilled Nursing Facilities (SNFs), serving as the critical link between patients, providers, and discharge planning teams.


Primary Responsibilities:

  • Conduct weekly SNF assessments using CMS criteria and clinical judgment.

  • Participate in interdisciplinary rounds and facilitate care conferences with patients and families.

  • Ensure timely discharge planning, care transition coordination, and goal tracking.

  • Collaborate with SNF providers, physicians, discharge planners, and rehab staff to remove discharge barriers.

  • Complete concurrent reviews and authorizations, manage referrals, and escalate cases to Medical Directors as needed.

  • Document all case activities and maintain HIPAA-compliant practices.

  • Support delegated contract start-up and cross-functional team coverage as needed.

  • Review analytics and dashboards to identify gaps and contribute to performance improvement.

  • Engage in weekly team meetings, continuing education, and adherence to care coordination policies.


Required Qualifications:

  • Active, unrestricted license in Kentucky as either:

    • Registered Nurse (RN)

    • Physical Therapist (PT)

    • Occupational Therapist (OT)

    • Speech-Language Pathologist (ST)

  • Minimum 5 years of clinical experience

  • Must reside within or near Mayfield, KY

  • Willingness to travel within 30 miles for onsite SNF coverage

  • Valid driver’s license and access to reliable transportation


Preferred Qualifications:

  • Experience with the geriatric population

  • Knowledge of care/utilization/resource management, home health, or rehab care

  • Experience with patient education and discharge coordination

  • Strong verbal/written communication, teamwork, and problem-solving skills

  • Familiarity with Microsoft Office Suite (Outlook, Excel, PowerPoint)


Work Conditions:

  • Requires home office setup for hybrid flexibility

  • Onsite work in SNFs up to 85% of the time

  • Ability to remain seated or mobile for extended periods

  • Frequent laptop and phone communication
