About the Role

Responsibilities

  • Provide comprehensive pharmaceutical care to long-term care patients.
  • Monitor and optimize medication regimens to ensure the best possible outcomes.
  • Collaborate with healthcare teams to ensure optimal patient care.
  • Administer immunizations and support health initiatives.
  • Ensure compliance with all relevant laws, regulations, and healthcare standards.

Qualifications

  • PharmD or MTM Certified.
  • Immunization and CPR Certifications.
  • Knowledge of long-term care pharmacy practices.
  • Strong communication and interpersonal skills.
