Bridging Care is dedicated to advancing health equity by providing innovative care coordination services that address the social determinants of health. As a partner of a non-profit affiliate, our mission is to improve access to healthcare and quality care for vulnerable populations.
We are looking for an independent and strategic Grant Writer to build and lead our grant writing program from the ground up. This role is critical to securing funding and establishing a sustainable grant program that will support our mission. The ideal candidate will be proactive, experienced, and skilled at crafting compelling proposals and cultivating relationships with funders.
