Position Title: Population Health Manager

Reporting To: Director of Managed Care

Location: AHS Family Health Center (AHSFHC)

Position Summary:
The Population Health Manager will lead and facilitate clinical improvement projects, assist with measuring and analyzing improvements, and contribute to reporting clinical outcomes. This role integrates improvement activities with regulatory and accreditation standards to ensure compliance and enhance patient care.

Duties and Responsibilities:

  • Program Leadership: Oversee the implementation and deliverables of population health programs within the clinic.
  • Stakeholder Engagement: Represent AHSFHC in meetings with external stakeholders related to population health initiatives.
  • Managed Care: Collaborate with health plans on managed care initiatives.
  • Quality Improvement: Apply quality improvement techniques to enhance customer care and clinical outcomes.
  • Reporting: Generate clinical performance and care gap reports, and implement measures to improve patient outcomes.
  • Health Education: Participate in health education and patient-facing initiatives offered by AHSFHC.
  • Regulatory Compliance: Integrate regulatory accreditation requirements into improvement projects.
  • Data Management: Abstract data from medical records for key clinical improvement projects.
  • Trend Analysis: Research healthcare quality improvement trends and lead the development of improvement initiatives.
  • Benchmarking: Assist in identifying benchmarks and comparisons for clinical outcomes measures.
  • Advisory Role: Advise and support leadership on quality and improvement matters.
  • Patient Satisfaction: Coordinate and implement patient satisfaction surveys across all clinic sites.
  • Certification Maintenance: Collaborate with AHSFHC CQI staff to maintain Patient Centered Medical Home (PCMH) certification.
  • Policy Development: Plan and prepare policies and procedures to meet NCQA’s PCMH accreditation standards.
  • Standards Achievement: Achieve and maintain regulatory and voluntary quality standards required for AHSFHC’s business practices.
  • Additional Duties: Perform other duties as assigned to support organizational goals.

Preferred Qualifications:

  1. Knowledge of managed care and healthcare practices.
  2. Experience in collecting, analyzing, and presenting statistical data.
  3. Effective oral and written communication skills with healthcare professionals and the general public.
  4. Previous experience in a healthcare setting with demonstrated ability to work in a multicultural environment.
  5. Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint).

Minimum Qualifications:

  1. B.S./B.A. in Social Work, Social Sciences, Health Sciences, or related field.
  2. Illinois RN License or MPH preferred.
  3. Valid driver’s license and insured automobile.
  4. Bilingual in an Asian language or Spanish preferred.
