Location: PharMerica (Nationwide)
Schedule: Mon-Fri (7 AM - 3 PM, 10:30 AM - 6:30 PM, 11:30 AM - 7:30 PM rotating) | Rotating Sat 8 AM - 4 PM & Sun 8 AM - 2 PM

Responsibilities:

  • Evaluate physician medication orders for appropriateness, dosage, potential interactions, and route of administration. Recommend therapy changes to physicians and facility staff as needed.
  • Maintain patient/resident databases, including medical history, clinical and laboratory data, and concurrent medications (including over-the-counter and home remedies).
  • Ensure appropriate laboratory tests are ordered and evaluated for high-risk conditions and collaborate with the healthcare team for follow-up.
  • Document all communication with healthcare team members and patients/residents in the medical record.
  • Implement and maintain policies for the safe procurement, storage, distribution, and disposal of drugs.
  • Maintain required documentation in accordance with federal and state regulations.
  • Report medication-related errors to the Pharmacy Manager.
  • Assist facility/agency staff with procedures relating to safe drug handling.
  • Perform other tasks as assigned.

Qualifications:

  • Bachelor’s Degree in Pharmacy (Pharm D preferred)
  • Minimum one year of experience in long-term care
  • Licenses/Certifications as required by government regulations
  • Detail-oriented, goal-driven, team player with strong oral communication skills
