Position Summary

As a Pharmacist in Charge, you will play a critical role in ensuring the highest level of clinical pharmacy services for hospice patients. Your responsibilities include providing therapeutic recommendations, medication management, and ensuring compliance with regulatory standards while maintaining a productive and error-free environment.


Key Responsibilities

Clinical and Consultative Services:

  • Offer clinical consultation and therapeutic recommendations to patients, nurses, physicians, and other hospice personnel.

  • Conduct medication profile reviews and histories, optimize regimens, and ensure safe pharmacotherapy practices based on pharmacodynamic and pharmacokinetic principles.

Customer Service:

  • Address patient and caregiver questions promptly and professionally.

  • Resolve customer issues with a focus on quality and satisfaction.

Operational Duties:

  • Transcribe, verify, and fill medication orders accurately.

  • Maintain proper storage and security of medications.

  • Ensure pharmacy workspace remains neat and orderly.

Team Collaboration:

  • Work cooperatively with pharmacy technicians and other pharmacists.

  • Foster a courteous and professional work environment, even during stressful situations.


Qualifications

Required Education and Licensure:

  • Doctor of Pharmacy (PharmD) degree, or BS in Pharmacy.

  • Current pharmacist license in good standing with the State Board.

Skills and Competencies:

  • Exceptional written and verbal communication skills, particularly over the phone.

  • Strong problem-solving skills with the ability to prioritize effectively.

  • Ability to work independently and maintain composure under pressure.

  • Team-oriented with a cooperative and professional attitude.
