Position Summary:

The Pharmacist Clinician plays a critical role in providing comprehensive disease management services, including anticoagulation therapy, hepatitis C, lipid management, smoking cessation, vaccinations, and more. The position involves assessing patient needs, planning and delivering care, coordinating services, and educating patients. The clinician may occasionally assume charge responsibility, serve as a preceptor for new hires, and act as a resource person for other staff. This role is essential in optimizing patient care and ensuring positive health outcomes.

Primary Responsibilities:

  • Patient Disease Management:

    • Assess, plan, deliver, and coordinate care for patients with various conditions, including anticoagulation, hepatitis C, lipid disorders, smoking cessation, and vaccinations.
    • Provide ongoing treatment and care based on patient needs, ensuring adherence to protocols and guidelines.
    • Recognize changes in patient status and adjust treatment plans accordingly.
    • Educate patients and families on disease management, medication adherence, and lifestyle changes.
  • Clinical Responsibilities:

    • Order and interpret diagnostic tests and therapies based on patient assessments.
    • Utilize prescriptive authority to initiate, modify, or discontinue treatment plans under established guidelines or protocols.
    • Monitor and manage medication therapy, ensuring the optimal pharmacologic care for patients.
    • Communicate with healthcare providers to optimize patient outcomes.
  • Collaboration and Communication:

    • Collaborate with physicians, nurses, and other healthcare professionals to ensure integrated care delivery.
    • Act as a resource for other staff, providing guidance and answering clinical questions.
    • Serve as a preceptor for new hires and pharmacy students.
  • Administrative and Supervisory Responsibilities:

    • May assume charge responsibility in the absence of the Pharmacy Director or Anticoagulation Management Services Coordinator.
    • Assist in the development and implementation of disease management protocols and standards.
    • Participate in quality improvement initiatives to enhance patient care and outcomes.

Minimum Qualifications:

  • Education:
    • Graduate of an ACPE-accredited School of Pharmacy.
    • Bachelor of Science in Pharmacy or Doctor of Pharmacy (Pharm.D.) required.
  • Licenses/Certifications:
    • Licensed Registered Pharmacist (RPh) in the State of New Mexico.
    • Licensed Pharmacist Clinician (PhC) by the New Mexico Board of Pharmacy.
    • Registered as a Pharmacist Clinician with the Board of Medicine to exercise prescriptive authority, in accordance with the Pharmacist Prescriptive Authority Act.
    • Certification in Basic Life Support (BLS) for Healthcare Providers.
    • Advanced training/certification in anticoagulation or disease management preferred.
  • Skills:
    • Proficient in gathering and analyzing physiological, psycho-social, and socio-economic data.
    • Ability to evaluate patient condition and make appropriate decisions based on changes in patient status.
    • Strong communication skills, both verbal and written; fluency in English required, fluency in Spanish preferred.
    • Ability to work in high-pressure environments, especially in stressful clinical situations.
    • Strong clinical pharmacy skills, including physical assessment, planning, and evaluation of care.
  • Experience:
    • Minimum of 2-3 years of direct patient care experience as a Pharmacist Clinician with prescriptive authority.
    • Experience in disease management is required, with knowledge and expertise in managing complex chronic conditions.

Physical Requirements:

  • Ability to bend, stoop, lift up to 20 lbs., and reach above head level or below knee level.
  • Ability to move around the clinic and interact with patients in various settings.
  • Potential exposure to physical injury, toxic substances, infectious diseases, and bloodborne pathogens. Knowledge of safety protocols and standards to minimize exposure is required.

Nature of Supervision:

  • Responsible to: Anticoagulation Management Services Coordinator and Pharmacy Director.

Environment:

  • Work in a clinical environment with exposure to health risks such as bloodborne pathogens and infectious diseases. Adherence to safety standards and precautions is essential.
