About the Position

This Medical Director position is for a House Calls Program delivering high-quality primary care to Medicare patients in their homes. The role offers flexibility with a primarily remote work structure but may require occasional office visits and patient home visits. As the Medical Director, you will provide clinical leadership, mentorship, and guidance to the care team, helping drive improvements in patient care and outcomes for older adults, particularly those in low-income communities.

Key Responsibilities

  • Leadership

    • Lead and mentor clinical teams, ensuring the highest standards of care and achieving care goals such as appropriate hospital admissions, preventive care, chronic disease management, and overall patient satisfaction.

    • Provide leadership to the team, including clinicians and administrative staff, and foster a positive team environment.

    • Coach providers on delivering excellent care to patients while maintaining a collaborative and supportive team atmosphere.

  • Clinical Responsibilities

    • Support the team in providing primary care for Medicare patients in their homes.

    • Provide ad hoc coverage for providers and attend patient visits as needed, though you will not manage a patient panel.

    • Lead by example in practicing thoughtful in-home primary care.

    • Develop and implement strategies to improve patient care and achieve quality outcomes.

  • Metrics and Strategy

    • Use a metrics-driven approach to measure the success of clinical care initiatives and quality improvement projects.

    • Collaborate with the Practice Manager to achieve the team's goals and refine operational strategies.

  • Mentorship and Development

    • Serve as a mentor and advisor for market providers, offering clinical guidance and leadership.

    • Focus on professional development and encourage team members to reach their potential.

Qualifications

  • Education & Certification

    • MD/DO required.

    • Board Certified in Internal Medicine, Family Medicine, or Geriatric Medicine.

    • Fellowship training in Geriatrics is a plus but not required.

  • Experience

    • 3+ years of experience in outpatient practice, with a preference for clinical leadership experience.

    • Prior executive roles, and experience with metrics-driven care or quality projects are welcomed but not required.

    • Experience in a leadership role, with a focus on team development and patient care quality.

  • Skills & Abilities

    • Strong leadership and mentorship capabilities, with a passion for supporting and developing teams.

    • Experience with value-based care models and achieving clinical targets.

    • Flexible, positive attitude with the ability to manage ambiguity and adapt to evolving situations.

    • Effective communication skills and the ability to work collaboratively across multidisciplinary teams.
