Location: Primarily Remote (with occasional in-person responsibilities)


Position Overview

The Medical Director is a leadership role focused on delivering high-quality, patient-centered primary care to Medicare patients. The role emphasizes mentorship, team development, and driving clinical excellence while fostering a supportive, mission-driven workplace culture.

This position does not involve maintaining a patient panel but includes ad hoc patient coverage, team leadership, and strategic oversight to achieve clinical and operational goals.


Key Responsibilities:

  • Clinical Leadership:

    • Lead and mentor market clinical teams, including providers, to achieve high standards of patient care and workplace satisfaction.
    • Serve as a role model for in-home primary care practices.
    • Drive team alignment on metrics for quality care, including hospital admission rates, preventive care, chronic disease management, and patient satisfaction.
  • Operational Oversight:

    • Collaborate with the Practice Manager dyad partner to implement strategies and initiatives aligned with organizational goals.
    • Support coverage needs, including ad hoc patient visits and team meetings.
  • Program Development:

    • Lead critical program-wide strategic initiatives and quality improvement projects.
    • Utilize a metrics-driven approach to monitor and enhance patient care and operational outcomes.
  • Team Development:

    • Mentor and coach providers to maximize their potential and foster a collaborative environment.
    • Promote a culture of learning and professional growth within the clinical team.
  • Patient Care:

    • Ensure excellent patient outcomes, focusing on keeping patients healthy and independent at home.
    • Occasionally conduct patient visits, either in the patient’s home or virtually.

Requirements:

  • Education & Certification:

    • Board Certified in Internal Medicine, Family Medicine, or Geriatric Medicine.
  • Experience:

    • Minimum of 3 years in outpatient practice (preferred).
    • Experience in clinical leadership roles, coaching teams, and driving performance improvement.
    • Familiarity with metrics-driven care delivery models.
    • Fellowship training in geriatrics, or advanced degrees (e.g., M.B.A., J.D., M.P.H.), welcomed but not required.
  • Skills & Qualities:

    • Strong mentoring and coaching abilities.
    • Flexibility and a positive attitude, including comfort with ambiguity.
    • Passion for quality improvement, team-building, and patient-centered care.

Care Delivery Model:

  • Team-Based Approach:

    • Each Care Team includes a provider, a medical assistant/scribe, a behavioral health specialist, and a mobile operations coordinator.
    • Patient panel of approximately 75 patients, visited monthly.
  • Goals of Care:

    • Minimize unnecessary hospitalizations.
    • Optimize preventive care and chronic disease management.
    • Deliver a superior patient experience, prioritizing quality of life, health, and independence.
