Job Summary (Healthcare Quality & Outcomes Focused)

Optum is seeking a Field Based Clinical Practice Performance Coordinator to partner with provider practices and quality teams to close HEDIS® and STAR rating gaps, improve preventive screening compliance, and optimize Medicare Advantage quality outcomes. You’ll review medical records, perform data collection & entry, coordinate appointments, support chart retrieval, and analyze quality opportunities—all while ensuring HIPAA compliance, population health impact, and patient satisfaction across high-volume UHC networks in the Boston region.


Key Responsibilities

  • Review medical records and highlight STAR/HEDIS quality opportunities for clinical teams.

  • Perform data collection, entry, image uploads, chart retrieval & quality monitoring.

  • Locate screening results/documentation to close care gaps (no clinical interpretation required).

  • Coordinate appointments (PCP/specialists), late-to-refill outreach, and local clinic event scheduling.

  • Conduct outbound & inbound calls to members and providers to facilitate care and chart collection.

  • Track, document, and report outcomes accurately across multiple systems/tools.

  • Partner with practice leadership to optimize workflows and remove barriers to closing care gaps.

  • Analyze reports (Excel) to identify open quality opportunities at member/provider level.

  • Maintain strict HIPAA compliance and secure PHI handling.

  • Support quality improvement campaigns to elevate plan/contract Star measures.

  • Travel (up to 75% as market needs dictate) to targeted provider sites.


Required Skills & Qualifications

  • High School Diploma / GED (minimum); 18+ years of age.

  • 1+ year of healthcare industry experience.

  • Proficiency with Microsoft Word & Outlook; ability to navigate, filter & analyze reports in Excel.

  • Working knowledge of HIPAA requirements.

  • Reside within one hour of Boston, MA.

  • Valid driver’s license & proof of insurance; ability to travel up to 75%.

  • Dedicated, private home workspace with approved high‑speed internet.

Preferred Qualifications

  • 1+ year working in a physician/provider/medical office.

  • EMR & HEDIS experience; knowledge of medical terminology.

  • ICD‑9/10 & CPT exposure (≥1 year).

  • Background as Pharmacy Technician, Medical Assistant, or LPN.

  • 2+ years of telephonic customer service in healthcare.
