Clinical Pharmacist - Mercy Health

Job Overview:
Mercy Health is seeking a Clinical Pharmacist to participate in clinical programs that promote safe and cost-effective medication therapy. The role includes a combination of clinical work, medication reviews, therapy management, and pharmacy operations. The Clinical Pharmacist will work in collaboration with healthcare teams to ensure optimal medication use, improve patient outcomes, and contribute to the success of clinical programs.

Essential Responsibilities:

  • Participate in clinical programs to enhance patient care, optimize medication therapy, and promote health and disease prevention.
  • Perform medication reviews and therapy management to ensure safe and effective medication use.
  • Support pharmacy operations for accurate medication distribution and availability.
  • Engage in cost-containment efforts related to medication utilization and pharmacy resources.
  • Communicate effectively with pharmacy teams, healthcare professionals, and leaders to provide high-quality pharmaceutical services.
  • Document and resolve adverse medication events, evaluating reports for prevention.
  • Provide education to patients and staff, serving as a preceptor for students and residents.
  • Maintain proficiency in order processing and assist with operational functions as needed.

Qualifications:

  • Licensing/Certification: Current pharmacist license in the state of practice, BLS preferred, ACLS preferred, Immunization Certified preferred.
  • Education: Graduate of an ACPE-accredited School of Pharmacy with a BS or PharmD. Advanced degrees such as Doctor of Pharmacy or Master’s preferred.
  • Experience: Advanced knowledge in pharmaceutical assessments and patient management. Residency or 3 years of equivalent experience preferred.

Physical & Working Conditions:

  • Exposure to potentially hazardous substances, body fluids, and chemicals.
  • Occasional travel to off-site locations.
  • Periods of high stress and fluctuating workloads.
  • Prolonged periods of working alone.
  • Physical demands include lifting, carrying, and repetitive arm/hand movements.

Skills and Competencies:

  • Strong knowledge of medications, adverse effects, interactions, and dosing.
  • Proficiency in pharmacy operating systems and automation tools.
  • Ability to develop SMART goals, manage databases, and interpret data.
  • Excellent communication, teamwork, and critical thinking skills.
  • Strong attention to detail, customer service orientation, and organizational skills.
