Clinical Pharmacist Position at David Grant Medical Center (DGMC), Travis AFB, CA

Overview:
The Clinical Pharmacist will provide comprehensive pharmaceutical care at DGMC, with responsibilities including patient counseling, medication management, and participation in clinical investigations. Healthcare workers (HCWs) will receive notice two weeks prior to reassignment within a 40-mile radius.

Education Requirements:

  • Doctor of Pharmacy (PharmD) from an ACPE-accredited institution, or
  • Bachelor of Science in Pharmacy (BS Pharmacy) plus one of the following:
    • Board of Pharmacy Specialties certification in recognized practice areas.
    • Completion of a clinical pharmacy residency or fellowship accredited by relevant bodies.
    • Post-Graduate Master of Science degree focused on medication therapy management.

Certifications:

  • Basic Life Support (BLS)
  • Advanced Cardiovascular Life Support (ACLS)

Experience:

  • Minimum one year of inpatient or hospital clinical pharmacy experience within the last five years.

Licensure:

  • Must hold an active pharmacist license in a U.S. jurisdiction, free from disciplinary actions.

Key Responsibilities:

  • Conduct clinical investigations and participate in outcome studies approved by the Investigational Review Board (IRB).
  • Counsel patients on medication use and ensure proper medication compounding.
  • Supervise inventory control and security of pharmaceuticals, especially controlled substances.
  • Respond to Code Blue situations and ensure compliance with USP 797 Sterile Compounding standards.
  • Precept pharmacy students and residents, facilitating their training and learning experiences.
  • Perform a full range of pharmacist duties, including:
    • Assessing patient responses to drug therapy and planning appropriate interventions.
    • Ordering and evaluating laboratory tests to monitor drug therapy effects.
    • Initiating or modifying medication regimens for chronic conditions.
    • Collaborating with other healthcare providers on patient treatment needs.
    • Conducting medication therapy reviews and ensuring accurate dispensing.

Work Schedule:

  • Pharmacy operating hours: Monday to Friday, 0600 - 2000.
  • Shifts will vary from 8 to 10 hours, starting between 0600 and 0900.

Compensation and Scheduling:

  • The schedule is maintained by clinic personnel and provided one month in advance. Alternative schedules may be implemented upon mutual agreement.

Training and Compliance:

  • Completion of required training, including Newcomers Orientation and annual updates on safety and compliance.

Healthcare Requirements:

  • Documentation of health requirements, including immunizations and tuberculosis screening, may be necessary at the time of placement.

Security Process:

  • HCWs must undergo security vetting, including fingerprinting and completing necessary forms for national security positions.
