Pharmacist (Per Diem) - Job Summary

This Per Diem Pharmacist role focuses on ensuring safe, efficient, and cost-effective drug therapies for patients while providing pharmaceutical care with compassion and excellence. Pharmacists will manage medication orders, monitor patient therapies, and educate patients and healthcare professionals.


Key Responsibilities

Operational Duties

  • Interpret and process physician drug orders, maintaining accurate computerized patient medication records.
  • Compound and dispense medications, including non-sterile/sterile products, chemotherapy, and parenteral nutrition.
  • Issue controlled substances and maintain compliance with legal and institutional protocols.
  • Supervise and verify the daily tasks of pharmacy technicians.
  • Maintain licensure and complete all required competency assessments and training.
  • Perform ambulatory pharmaceutical care in specialized areas (e.g., anticoagulation clinics, transition of care).

Clinical Duties

  • Monitor drug therapy for interactions, contraindications, and appropriateness.
  • Participate in pharmacokinetic consultations and renal drug dosing adjustments.
  • Provide drug information and education to patients, families, and healthcare staff.
  • Engage in quality improvement activities, data collection, and clinical interventions.
  • Support stewardship programs for antibiotics, opioids, and medication shortages.
  • Attend interdisciplinary rounds and provide clinical consultations as needed.

Qualifications

Education & Skills

  • Required: Doctor of Pharmacy (Pharm.D.) or Bachelor of Science in Pharmacy.
  • Strong communication and clinical skills are essential.

Experience

  • Hospital pharmacy experience preferred.

Licensure & Certifications

  • Pharmacy registration in the state of practice required.
  • State licensure must be obtained within 60 days of hire.
  • Required: BLS or ACLS certification (PALS for pediatric practice) within 180 days of hire.

Occupational Hazards

  • Exposure to hazardous/toxic substances, sharp objects, and biologic material.
  • Risk of musculoskeletal injuries without proper lifting techniques.

Work Environment

This is a Per Diem position with flexible scheduling as needed.
