Job Summary

Optum’s naviHealth team is seeking an experienced Care Coordinator (RN, PT, OT, or ST) to support post-acute care (PAC) transitions and improve patient recovery outcomes. This role involves conducting skilled nursing facility (SNF) assessments, engaging care teams, and ensuring seamless transitions across care settings to support older adults’ healthcare journeys.


Key Responsibilities

  • Act as the primary link between patients, families, and healthcare teams for smooth care transitions.

  • Perform SNF functional assessments and review CMS criteria to ensure quality care.

  • Monitor patients’ progress, collaborate on discharge planning, and address barriers.

  • Participate in weekly SNF rounds, presenting accurate patient updates.

  • Coordinate referrals to Health Plans, Case Managers, and community services.

  • Attend patient/family care conferences and maintain updated documentation.

  • Conduct utilization management reviews and coordinate peer-to-peer physician reviews.

  • Support new contract start-ups and cross-functional coverage as needed.

  • Review clinical dashboards, readmission reports, and identify process improvements.

  • Adhere to HIPAA and all federal, state, and organizational policies.


Required Skills & Qualifications

  • Active, unrestricted license as Registered Nurse, Physical Therapist, Occupational Therapist, or Speech Language Pathologist.

  • 5+ years of clinical experience in healthcare.

  • Must live within 30 miles of Fairlawn, OH and have reliable transportation.

  • Strong knowledge of care coordination, utilization management, and disease management.

  • Proficiency in Microsoft Office (Excel, Outlook, PowerPoint).

  • Exceptional communication, problem-solving, and decision-making skills.


Preferred Qualifications

  • Experience with the geriatric population and rehabilitation or home health nursing.

  • Background in patient education and post-acute care processes.

  • Ability to manage high caseloads and multiple priorities effectively.
