💡 Job Overview

The Ambulatory Clinical Pharmacist provides optimal pharmaceutical care to patients through both clinical and distributive functions. This role collaborates with healthcare teams to ensure safe, evidence-based, and cost-effective medication management. As the clinical pharmacy liaison between specialty pharmacy, specialty clinics, and patients, the pharmacist actively contributes to direct patient care, clinical documentation, and quality improvement initiatives.

📌 Key Responsibilities

  • Clinical Collaboration: Work closely with physicians, nurses, and healthcare teams to provide comprehensive medication management services.
  • Medication Management: Review drug therapy for appropriateness, identify safety concerns, and ensure compliance with best practices.
  • Patient Care: Conduct medication reconciliation, provide drug therapy recommendations, and educate patients on medication use.
  • Regulatory Compliance: Ensure adherence to Ohio Board of Pharmacy regulations, TriHealth policies, and national treatment guidelines.
  • Process Improvement: Identify opportunities for efficiency, cost reduction, and enhanced patient safety in pharmacy operations.
  • Drug Distribution & Inventory Management: Verify prescriptions, supervise technicians, adjust par levels, and prevent medication waste.
  • Professional Development & Leadership: Act as a preceptor for pharmacy students and residents, participate in committees, and support team training.

📋 Job Requirements

  • Education: Bachelor’s Degree in Pharmacy
  • Licensure: Registered Pharmacist with an active Ohio Board of Pharmacy license
  • Experience: 1-2 years in Pharmacy (experience in Inpatient, Oncology/Infusion, Specialty, or MTM preferred)
  • Skills: Strong analytical, organizational, and problem-solving abilities; ability to collaborate with diverse multidisciplinary teams
