Job Details

This position primarily serves the Endocrine and Diabetes clinic and involves direct patient care and collaboration with other healthcare providers. A strong clinical pharmacy background with an understanding of retail pharmacy operations and prescription processing is optimal to succeed in this role. The role is 100% on-site in West Reading, Pennsylvania. Cross coverage of other specialty areas may be required.


Hours

  • Monday through Friday day shift

  • Typically no weekends or holidays required


Responsibilities

  • Demonstrate value of ambulatory pharmacy partnership to physician practices.

  • Maintain presence in clinic, field medication questions from providers such as formulary product selection, dosing, adverse drug reactions (ADRs), and other considerations.

  • Ensure prescriptions flow through ambulatory pharmacy and are identified by pharmacy staff.

  • Maintain patient schedule for clinical pharmacist services including medication counseling, pillbox refills, immunization, medication administration.

  • Maintain patient list, schedule appropriate follow-ups to assess medication safety and efficacy, communicate treatment plan recommendations with providers.

  • Coordinate patient refills; ensure auditable records showing proof-of-delivery are maintained.

  • Prepare and submit prior authorizations, manage denials and appeals.

  • Triage prescriptions that are limited distribution drugs or out-of-network.

  • Act as financial advocate for patients, leveraging charitable foundations, state programs, and manufacturer programs to ensure medication affordability.

  • Demonstrate ability to navigate various platforms and document appropriately in EMR and specialty therapy management platforms.

  • Contribute to expansion of ambulatory pharmacy services through active participation in navigator committee and projects.

  • Cross-train to ambulatory pharmacy, assist with staffing as needed.


Education

  • PharmD degree required.

  • PGY1 residency or previous experience in ambulatory care and diabetes management highly desirable.

  • Unrestricted pharmacist license (including immunization) in the state of Pennsylvania is required. If out of state, licensure must be obtained within 90 days of offer acceptance.

  • Experience with 340B purchasing program is beneficial.


Skills

  • Proficiency in Microsoft Office (Word, Excel, PowerPoint).

  • Experience in Epic or similar EMR platforms.

  • Experience with retail pharmacy prescription processing and claims adjudication.

  • Comfortable working with clinical data, lab values, and patient medical records.
