Job Details
This position primarily serves the Endocrine and Diabetes clinic and involves direct patient care and collaboration with other healthcare providers. A strong clinical pharmacy background with an understanding of retail pharmacy operations and prescription processing is optimal to succeed in this role. The role is 100% on-site in West Reading, Pennsylvania. Cross coverage of other specialty areas may be required.

Responsibilities
Demonstrate value of ambulatory pharmacy partnership to physician practices.
Maintain presence in clinic, field medication questions from providers such as formulary product selection, dosing, adverse drug reactions (ADRs), and other considerations.
Ensure prescriptions flow through ambulatory pharmacy and are identified by pharmacy staff.
Maintain patient schedule for clinical pharmacist services including medication counseling, pillbox refills, immunization, medication administration.
Maintain patient list, schedule appropriate follow-ups to assess medication safety and efficacy, communicate treatment plan recommendations with providers.
Coordinate patient refills; ensure auditable records showing proof-of-delivery are maintained.
Prepare and submit prior authorizations, manage denials and appeals.
Triage prescriptions that are limited distribution drugs or out-of-network.
Act as financial advocate for patients, leveraging charitable foundations, state programs, and manufacturer programs to ensure medication affordability.
Demonstrate ability to navigate various platforms and document appropriately in EMR and specialty therapy management platforms.
Contribute to expansion of ambulatory pharmacy services through active participation in navigator committee and projects.
Cross-train to ambulatory pharmacy, assist with staffing as needed.
Education
PharmD degree required.
PGY1 residency or previous experience in ambulatory care and diabetes management highly desirable.
Unrestricted pharmacist license (including immunization) in the state of Pennsylvania is required. If out of state, licensure must be obtained within 90 days of offer acceptance.
Experience with 340B purchasing program is beneficial.
Skills
Proficiency in Microsoft Office (Word, Excel, PowerPoint).
Experience in Epic or similar EMR platforms.
Experience with retail pharmacy prescription processing and claims adjudication.
Comfortable working with clinical data, lab values, and patient medical records.
