Pharmacist (Outpatient)

Location: Salem Health, Salem, OR
Salary: $87K-$107K per year (estimated)
Position Type: Full-Time
Start Time: 7:00 PM


Overview:

Join Salem Health as a Pharmacist in the Outpatient Pharmacy department and contribute to a pioneering healthcare organization that has been serving the community since 1896. Key Responsibilities:

  • Medication Management: Evaluate and verify provider medication orders for appropriateness, dosage, potential interactions, and route of administration. Recommend therapy changes as needed.

  • Patient Care: Monitor and manage medication therapy for hospitalized or ambulatory patients. Coordinate with healthcare providers to ensure optimal therapeutic outcomes.

  • Collaboration: Participate in Unit Care Rounds and consult with medical providers about medication therapy.

  • Supervision: Oversee pharmacy technicians, pharmacy students, interns, and residents.

  • Medication Safety: Ensure accuracy in medication preparation, verify correct administration, and perform inventory management.

  • Patient Education: Provide counseling on medications and collaborate with physicians to resolve any issues related to drug therapy.


Required Qualifications:

  • Education: Doctor of Pharmacy (PharmD)

  • Licensing: Oregon Licensed Pharmacist required

  • Certifications: BLS and ACLS required

  • Experience: PGY1 Pharmacy Residency preferred
