This role supports an integrated care model, ensuring that members receive personalized, coordinated care across the continuum. The RN Clinical Care Manager acts as a primary advocate for members, ensuring that their clinical, social and community-based needs are met effectively.

A $7,500 sign-on bonus is available for external applicants.


Position Overview

The RN Combination Clinical Care Manager is responsible for assessing, coordinating and managing care for Medicaid members with complex medical and social needs. This field-based hybrid role requires regular travel within Queens, New York. When not traveling, the RN will work from home.

Candidates must reside in New York State.


Key Responsibilities

  • Conduct comprehensive assessments, including the New York State UAS assessment, in member homes at least twice annually or as required.

  • Develop personalized care plans focused on achieving the most appropriate and least restrictive level of care.

  • Coordinate referrals for community, financial, psychosocial and state-supported services.

  • Serve as the primary point of contact for members throughout their care continuum.

  • Ensure timely communication with providers, interdisciplinary teams and care partners to maintain quality, coordinated care.

  • Advocate for patient needs and preferences to ensure support from the care team.

  • Maintain accurate documentation in electronic charting systems.

  • Participate in ongoing training, quality activities and performance improvement initiatives.


Required Experience and Qualifications

  • Active, unrestricted Registered Nurse (RN) license in the state of New York.

  • Minimum 2 years of clinical experience in a healthcare setting.

  • Minimum 1 year of community-based case management experience working with populations requiring complex care.

  • Experience in long-term care, home health, hospice, assisted living or public health.

  • Proficiency in Microsoft Word, Excel and Outlook.

  • New York State ID (or ability to obtain before hiring).

  • Residency in New York State.

  • Ability to travel throughout the assigned region to visit members in homes, hospitals, community centers or provider offices.


Preferred Qualifications

  • Behavioral health or other clinical specialty degree.

  • Experience with UAS-NY.

  • Familiarity with electronic medical documentation systems.

  • Background in managing medically or behaviorally complex populations.

  • Prior field-based work experience.

  • Experience arranging community-based support services.

Compensation and Benefits

  • Hourly Pay Range: USD 40.00 – 54.00 (based on full-time schedule and experience).

  • Additional compensation may reflect education, certifications and market conditions.
